Contract of Service

KNOW ALL MEN BY THESE PRESENTS

This Contract made and entered between:

Department of Education, Mayana National High School, a government agency with office address at

Mayana, City of Naga Cebu, herein represented by Kim C. Lawi-an, the designated school head

hereinafter referred to as th “First Party”

-and-

Vivian O. Canlom, of legal age, Filipino, and with residence of Mayana, City of Naga Cebu referred as

the “Second Party”

1. The first party is in need of the service of the second party who shall performed the functions

that cannot be performed by the regular personnel of the first party.

2. That the second party has signified her intention, to which the first party has accepted to

provide the service needed by the latter.

3. The second party herby possessed the experience and skills required to perform the job as

described herein.

4. That the second party hereby attest that she is not replated within the third degree on

consanguinity or affinity to the 1. Hiring authority, 2. Representative of the party that she has

not reach the compulsory age of sixty-five.

5. That in view hereof, the second party is herby contracted as School Utility for the period of

one year in consideration of the of monthly honorarium of Two Thousand Five Hundred Pesos

(P2500) which will be paid on monthly basis. This contract shall take effect on February 2021

to February 2022.

6. That as a utility personnel, the second party is expected to perform the following functions:

a. Clean the school premises and office of the school head.

b. Assist in cooking food for the school feeding program.

 c. Assist in planting the vegetable in Gulayan sa Paaralan Program

d. Other as may be deemed necessary

7. That the second party shall perform work at a time and scheduled agreed upon by both

parties.

8. It is agreed that the second party is required to render work Monday thru Fridays; in the

event that the second party fails to render the service contracted upon in the prescribed days

and time, the second party is required to render additional day to cover up her absences

without additional renumerations.

9. That it is understood that this contract does not create any employee-employer relationship

between the first and second party, that the services rendered hereunder are not considered

and will not be accredited as government service; and that the latter is entitled to benefits

enjoyed by the regular personnel of the first party.

IN WITNESS WHEREOF, both parties have hereunto set their hands on his 10 th day of

February 2021 at Mayana City of Naga Cebu.
