Subject: Application for Sick Leave

Respected Sir/Madam,

I hope this letter finds you in good health. I am writing to request a leave of absence due to
personal health reasons that have been affecting my mental well-being. I have been experiencing
significant amount of stress and depression, which has impacted my ability to perform
academically.

Therefore, I kindly request sick leave from my BS English classes for the 5th semester, from the
7th of March 2024 to the 9th of March 2024. This short break will allow me to focus on my well-
being and return to my studies with full dedication and improved health. I assure you that I will
take all necessary measures to catch up on any missed coursework and lectures upon my return.
