Student ministers are students. They are in the process of learning to minister. The opportunity
to make mistakes is an important part of that process. The purpose of this form is to facilitate the
student’s growth as a minister by becoming the basis of a reflection paper. One copy of this
form is to be completed by the ministry supervisor. It is to be mailed to the director of field
work in the envelope provided. The director of field work may call the supervisor to further
discuss the student’s strengths and needed areas of growth. Please evaluate the student’s
