A RESOLUTION TO CHANGE SIGNATORIES OF BARANGAY CHAIRPERSON/TREASURER FOR THE

ACCOUNT OF BARANGAY BURGOS MANGATAREM, PANGASINAN WITH ACCOUNT NUMBER
________ IN LAND BANK OF THE PHILIPPINES - MANGATAREM BRANCH.

WHEREAS, the Sangguniang Kabataan on its special session presided by Sangguniang

Kabataan Chairperson, Arlha Marie Viray Sarmiento presented to the body the need to change
signatories due to ______

WHEREAS, in order to facilitate Barangay fund Disbursement, the Sangguniang Barangay has to
change signatories in the name of Barangay of Burgos with account number _____ in Land bank
of the Philippines - Mangatarem Branch;

NOW THEREFORE, on motion of SK Councilor ALICIA NICOLE N. CARBONEL and duly seconded
by SK Councilor EDICEL C. FABROS, it was;

RESOLVED, as it hereby resolved to change signatories of Current Account with Land Bank of the
Philippines - Mangatarem Branch in the name of "Barangay of Burgos" to set up the NTA
Account of the Sangguniang Barangay designating HON. ARLHA MARIE VIRAY SARMIENTO, SK
Chairperson and MR. ICON JAMES PAGARIGAN, SK Treasurer, as authorized signatories of the
account.

RESOLVED FURTHER, that the Resolution remains effective and subsisting and has not been
amended, revoked or suspended and copies of this resolution be furnished to the Land Bank of
the Philippines - Mangatarem Branch and the Municipal Accountant Mr. KENAZ W. BAUTISTA for
their information and guidance.
