The following questions must be answered by the Inductee before commencement on site

Do you suffer from epilepsy or asthma?

Are you taking any prescription drugs which might affect your ability to work safely?

Are you under 18, and if so, has a risk assessment been undertaken for your proposed activities?

New Starter & Visitor Site Induction Form

This form is to be kept on site and made readily available for the duration of the project. If the Inductee leaves site
and returns when the site has changed significantly, then he/she must be re-inducted.

o Anyone wilfully damaging welfare facilities or construction works will be liable to instant dismissal.

o All accidents or dangerous occurrences on site must be immediately reported to the Site Manager, and

entered into the Accident Recording preform.

o Scaffolding must not be altered by anyone other than competent site scaffolder. If alteration is required

report to the Site Manager.

o Tower scaffolds must be constructed correctly by a competent person. (PASMA) complete with guardrails,

toe boards must be fitted and access up the tower must be internal.

o Ladder access must be safe and secure. Whenever ladders are used they must be secured or footed whilst

in use.

o All electrical tools must be 110V and current test certificates complying with current Electricity at Work

Regulations.

o Abrasive wheel/stone saws must only be changed and operated by certified and authorised operatives. This

certificate must be available on request.

o All plant operators must hold recognised Certificates of Training Achievement and these must be available

at all times.

o This site is a; safety helmets, hi viz & boots must be worn by everyone at all times. Anyone found not

wearing their safety helmet or boots may be liable for instant dismissal.

o Gloves will be worn as appropriate to the risk, as per contractors Risk assessments and method statements

(RAMS)

o A permit must be obtained prior to any; hot works; excavations; electrical works; roof works being carried

out on this project.

o All personnel must ensure that they are aware of the risk factors appertaining to their job and that they have

received adequate job specific training for the task which they have been employed to carry out on this site.

o All subcontractor personnel must have read and signed their method statements & risk assessments before

commencing any work onsite.

If you have any suggestions or concerns concerning the Health & Safety of the site please do not hesitate to talk to the site

manager.

We intend everybody on site to have and active role in maintaining a safe working environment.

I have attended a site induction meeting for this project.

I have declared to the Site Manager any pre-existing conditions or illnesses that may cause a danger to myself or

others.

I agree to and have read the Site Rules set out by ISS and understand that if I do not abide by these rules I may be

liable for instant dismissal from this project.
