AFFIDAVIT OF CORRECTION

I, POLICARPIO CAPUCHINO BATAIN, of legal age, married, Filipino citizen

and with residence and postal address located at No. 822 Purok 6, Barangay Malitlit,
City of Santa Rosa, Laguna, after having been duly sworn to an accordance with law, do
hereby depose and state the following:

1. That I am the spouse of MARIA LUISA CASTRILLO BATAIN, a CO-BORROWER

for a loan/mortgage offered by PAG-IBIG HOME DEVELOPMENT MUTUAL
FUND;

2. That on the Loan and Mortgage agreement which was executed on January 18, 2024,
there was an erroneous entry of my name;

3. That on the above-mentioned name, my undersigned name was erroneously

entered as “MICHAEL ESCUDERO ORTEGA” instead of POLICARPIO
CAPUCHINO BATAIN;

4. That POLICARPIO CAPUCHINO BATAIN instead of MICHAEL ESCUDERO

ORTEGA is true and correct and request such to be indicated or declared in the
above-mentioned documents;

5. I am executing this affidavit to attest of the truth of the foregoing facts;

6. I am further executing this affidavit as a compliance of the requirements of the

Registry of Deeds for Province of Laguna, Calamba Branch, for the whatever legal
purposes it may service.
