specifications AND local regulation.
› The operator must inspect the plant before start
works on daily basis, then deliver these inspection
forms to the plant/equipment coordinator of his
employer (weekly).
› The operator must use all mandatory PPE (helmet, ear
protection, safety glasses, high visibility vest, gloves
and safety shoes)
› A maintenance/repair logbook must be available in
the cockpit at all times. It must be written in a
language the operator can read and understand.
› The operator’s employer must ensure that the
operator is physically fit for the position, and that
he/she is technically competent to operate the
plant/equipment.

This form must be filled before the plant/equipment is authorised to enter the project premises. All inspection items will be inspected by H&S Team before access is granted to site. Plan/equipment will be authorised
only if all items are marked as “good”. Otherwise, the owner/supplier must repair/replace all those items marked as “bad” (H&S Team is not responsible for any loss time or production suspension due to
plant/equipment being rejected because of its bad conditions). Once all defects have been corrected and verified, a permanent pass (sticker located on a visible place) or a temporary pass (a card located on a visible
place) will be delivered to the operator as required. The operator must attend induction training, use all mandatory EPP and follow any instructions given by supervision or H&S staff.
