Dear Sir Mike,

We would like to inform your good office at around 9:00 in the morning we have one
(1) worker (John Paul Decera) helper in our soil boring group located to Banyan Tree
villa 1 road 5. While Mr. Dacera removed junction hose of soil boring machine, Safety
officer Francis Reyes Jr. saw that he wasn’t wearing a hardhat, he called him, but he
didn’t answer and didn’t come, Mr. Dacera said he was just finishing what he was
doing and he said we would come, but Sir francis Reyes called him again just to tell
him and make him wear his hardhat. Mr. Dacera spoke in a whisper/softly, he said,
just punch a hole in my id.

We do not condone such bad behavior because it can be imitated and done by
others.

Action taken:

We immediately talked to him and made him understand that such behavior is wrong
and should not be done to any person who can talk to him.
We also gave him a last chance, so that he could continue working and correct what
he did wrong, and at the same time, gave him last warning that if he repeats, he will
be fired immediately.

We apologize for what happened, we will do our best to prevent it from happening
again and not to be imitated by others.
