1. SCOPE OF THE ACTIVITY:


1.1. Work Permit Preparation
1.2. Corrective repair works includes the following:
1.2.1. Replacement of corroded pipes
1.2.2. Replacement of corroded flangee
1.2.3. Replacement of corroded fittings(elbow, tee)
1.2.4. Perform repainting on newly repair parts.
1.3. Cleaning of work place.

2. DUTIES AND RESPONSIBILITIES:


2.1. The Contracted Service Provider shall conduct Safety Inspection of the area.
2.2. Must attend the Safety Orientation prior to the start of the activity.
2.3. Comply with all other safety, permit-to-work procedure, lock-out and tag-out procedure and environmental instructions.
2.4. Must report to Safety Group before and after the activity
2.5. Conduct House keeping and disposal of debris. Ensure no left over debris on the area
2.6.Upon completion of tasks or activities, the service provider head will inform
2.7. Must submit their Work Completion Report and Certificate of Acceptance/Completion once completed
2.8 Contracted service provider shall secure the appropriate Gate Pass, Entry Pass and Vehicle Pass.
2.9 Contracted service provider personnel shall have their own Supervisor and Safety Officer.
2.10 Contracted service provider personnel shall wear the prescribed working clothes.
2.11. Contracted service provider shall provide the equipment necessary to conduct the activity (i.e. Ladder. Scaffold, Lift, etc.)

TERMS AND CONDITIONS


PRICING: In Philippine Peso (VAT Inclusive)
PAYMENT TERMS: 50% Down Payment Upon P.O
50% Full Payment upon received of Sales Invoice and PM Report.
PENALTY CHARGE: A penalty charge of 20% of the total investment cost will be imposed for the
Cancellation of Purchase Order.
VALIDITY: Prices are valid for 30 days upon receipt of this proposal.
VARIATION ORDER: Should there be any changes or addition in the design, item specifications
with regard to the project; it shall be treated as a variation to the agreement. shall
Any cost that is directly attributed to the change/s shall be to the account
of the client
Note: All defective parts that are subjected for replacement should be replace and will be quoted separately.
