PENDING REQUIREMENT AGREEMENT

Grade ___ student for S.Y. 2022-2023, recognize my/our duties and responsibilities regarding the

submission of all Enrollment Requirements to NEU-IS Grade School / JHS / SHS Program.

I/We acknowledge that my/our child’s enrollment is provisional to his completion of the requirements
enumerated below and shall be made official when I/we submit to the school the original copy of the
documents on or before September 16, 2022.

IMPORTANT:
1. Failure to submit on the said date may forfeit my enrollment in NEU-IS Grade
School / JHS / SHS Program without any refund.
2. Release of my Transfer Credentials and Official Transcripts will depend on my full
payment of any remaining balance to NEU-IS Grade School / JHS / SHS Program.

Guidelines on the Submission of Enrollment Documents for S.Y. 2022-2023

1. Pack the original copy of the following documents in a long brown envelope:

• SF9 / F-138 (Learner’s Progress Report Card)


• PSA-issued Birth Certificate
• Certificate of Good Moral Character (CGMC)
• Certificate of INC Membership/Patotoo ng Local (CINCM) [For INC Members Only]
• Parent’s Letter of Intent to Study at NEU [For Non-INC only]
• Endorsement Letter [from the INC Local Congregation in the place of residence [For
Non-INC only]
• Passport [For Foreign Nationality or Students From Foreign Schools Only]
• Special Study Permit from the Bureau of Immigration [For Foreign Nationality or
Students from Foreign Schools Only]
• ESC Certification with ESC ID No. & ESC School ID [For Education Service Contracting
Grantee Only]
• QVR Certification with Serial No. [For Senior High School Qualified Voucher Recipient
Only]

2. Seal and then label at the back of the long brown envelope with the proper:

• Complete Name
• Student Number
• Grade Level, Strand, and Section
• Mobile Number
• Date of Submission
• List of Documents submitted
3. Address the package with the following:

• Mobile Number
• Student Number (AdCon #)
• Grade Level / Strand / Section
• Date of submission / Courier / Tracking number
• Documents submitted

5. Strictly observe the deadline of submission:

• Send the enrollment documents on or before September 16, 2022, only.
