1 GENERAL

1.1 Purpose and Scope

The scope of this procedure to describe Clearing and Grading work of area Pertamina Hulu
Energi at the location of the Gas Metering Station-Grissik Metering System, PT Pertamina
Hulu Rokan Regional 1 Zone 1 Jambi Merang Field and PERTAGAS Metering Station-
Grissik.

1.2 Definitions and Abbreviation

3 SAFETY & HEALTH

 All personnel shall attend safety induction prior to entering site.


 All personnel shall have valid Brown Card.
 All personnel shall attend safety induction of the safe work practices and procedures to be
followed when working on or near the vicinity of high voltage electrical transmission line (High
Tension Cables).
 All HSE issues in every work location will be coordinated by the Safety Officer assisted by Safety
Representative.
 All personnel shall be provided with and shall use the appropriate personal protective equipment
(PPE) (e.g. hard hat, safety goggle, face shields, dust masks, safety shoes, hand gloves, rubber
boot, reflector vest, etc.).
 All tools and equipment shall be checked daily by qualified person prior to usage.
 Any damaged equipment or tools shall be not used. They shall be tagged, removed from site
and replaced by equipment or tools in proper condition.
 A Job Hazard Analysis (JHA) shall be prepared by supervisor and be attached to the permit to
work (PTW) for COMPANY site representative approval. JHA shall be developed in accordance
with Risk Assessment.

 The JHA shall be clearly explained to all workers by the supervisor during the tool box meeting.
Supervisor shall make sure that all personnel understand all the risks and hazards identified in
the JHA.
 All personnel involved in operations in close proximity to high voltage power lines shall be made
aware of the conditions & hazards around them. The hazard shall be included in the JHA.
 All personnel shall be made familiar with the Emergency Response Plan (ERP)
 All required warning signs, barriers and protection shall be installed prior to execution.
 Any operations carried out where there is public access or a thoroughfare such as a road, shall
be closely monitored by the Site Safety Officer.
 First aid kit and fire extinguisher shall always be at site and ready to use. Fire extinguishers
shall be fully charged and latest inspection tag shall be exhibited.
 First aider (trained personnel on crew spread) shall be made available on site at all time.
 The work shall be suspended during heavy rains or thunderstorms.

4 SECURITY

 Security will be sourced from the local villagers. They will attend a security induction for the
security system that will be implemented.
 The security shall consist of minimum 3 persons at each spread (1 person at day shift, 2 persons
at night shift). The security team is responsible for the security of the local villagers and they
shall patrol all working areas of their spread.
 Security will be provided at all times when equipment, tools and material are left at site without
any working activity. Security shall patrol the job site regularly to keep the working area free from
non-authorized persons and local community, and to prevent property loss.
 Badge/verification card shall be used by all personnel during working time on site.
 Site security shall be report to the Safety Officer if any incident occur.

5 ENVIRONMENT

 Workers shall keep the working area clean and tidy. Wastes shall be disposed of in disposal
bins.
 While filling equipment with fuel, manual hydraulic pump shall be used. Plastic sheet and drip
pan shall be placed under the equipment to manage fuel spill.
 All vehicles associated with refueling operations or the transportation of bulk petroleum product
in support of push pull and swampy area construction activities will be equipped with a spill
response kit designed to manage of small spills of fuels, lubricants, and other chemical materials.
The spill kit will include absorbent materials, shovels, rubber gloves, and large plastic bags to
contain saturated absorbents and contaminated materials.
 Smoking at work site area is strictly prohibited.

6 REGULATION PERMITS

6.1 Regulatory Permit

Prior to start construction activities, the supervisor shall ensure that the site has been
permitted to work by COMPANY, local authorities and/or local land owner to avoid
unnecessary delays due to any problems that may arise during execution.
Valid permit from PHE-JM and/or Local authorities must be obtained to work around the
vicinity of high voltage electrical transmission line that is considered hazardous or potentially
risky.

6.2 Working Permit

Permit to Work (PTW) shall be prepared by Supervisor and approved by COMPANY


representative. A permit-to-work coordinator will ensure each permit are filled out correctly
with necessary attachment (certificates, JHA, etc.). PTW from Owner only applicable when
working limited inside the PHE-JM ROW only.

7 MANPOWER

CONTRACTOR will mobilize the following manpower to perform Clearing & Grading work. Each
team shall minimum to consist of the following:

Un-skilled manpower or workers (helper) will be recruited from the local community. Skilled workers
will be recruited from local source as a primary source, and out of local district skilled workers will
be recruited when necessary. All workers shall attend a safety induction by the Safety Officer and
shall be briefed about the working hazards and risks prior to being assigned to work.

8 ROLES AND RESPONSIBILITIES

8.1 Supervisor
Supervisor will be responsible for the overall coordination and supervision of the clearing and
grading work. Supervisor will provide necessary information and liaise with PTW Coordinator
and shall be responsible for applying the correct work permits prior to commencement of
works. Supervisor shall also be responsible for the overall implementation of the Project HSE
Plan and safety regulations that govern.

8.2 Surveyor
The Surveyor is responsible to perform and supervise the survey activities. Surveyor is also
responsible to collect data of actual site conditions. The Surveyor shall submit the collected
as-built Survey data to the Field Engineer for further data processing.

8.3 Receiving Authority (RA)


A person authorized to receive approved permits and shall be responsible for supervising the
execution of the work by Permit Work Leaders as per the precautions in the permit. The
Receiving Authority (RA) can also be the Permit Work Leader (PWL). The Receiving
Authority (RA) shall also specify controls for safe work and requirements of other certificates.

8.4 Safety Officer


The Safety Officer will be responsible for the implementation of the HSE Plan and in
compliance with all statutory regulations that govern on all the activity commenced at his
assigned spread. The safety officer will be assisted by a safetyman at every construction
spread to help supervise the HSE aspect of the work and also to conduct safety briefings.

8.5 Excavator / bulldozer Operator


The excavator / bulldozer operator is responsible to operate the heavy equipment correctly
and safely. The operator shall be certified & have adequate knowledge about the ground
conditions to avoid the equipment getting bogged down on site especially in swampy areas.
The operator shall be always be accompanied by a watcher which will monitor the equipment’s
movement and look out for the safety of personnel as well as for any existing properties that
could be damaged by the operation.

8.6 Helpers
The Helpers are responsible for performing all work ordered by supervisor correctly and
safely. Work will consist of unskilled activities such as: preparing the hand tool, perform
manual handling,

8.7 QC Inspector
The QC Inspector is responsible for the quality of the work produced by the construction
team, The QC Inspector shall report to the QA/QC Department Manager.

8.8 Flagmen
The Flagmen are responsible to manage the traffic during the working activity especially
during equipment movement. The Flagmen are also responsible for the implementation of
traffic management plan.

8.9 Driver
The Drivers are responsible for driving the vehicle safely. Drivers shall have valid driving
license. The Drivers are also responsible for the state of the vehicle that they drive.

9 EQUIPMENTS

CONTRACTOR will mobilize the following equipment to perform the clearing & grading activities.
Each team will consist of the following:

10 WORKING TIME

Working times will start at 8.00 am, after a Pre-Job meeting and a check that all equipment and tools are
in good condition prior to use. Lunch time is 1.00 to 2.00 PM. Working continues at 2.00 PM until 6.00 PM.
Prior to leave the working area, all equipment and tools shall be tidied up and the working area shall be
cleaned. Supervisor shall inform the safety officer and COMPANY site representative when overtime is
required, in a timely manner. No work during rainy and risky weather.

11 PERMIT TO WORK

 Prior to commencement of clearing and grading work, the CONTRACTOR shall submit an application
for permit to work to the COMPANY in accordance with the CONTRACTOR’s Permit to Work system
to be agreed by the COMPANY.
 CONTRACTOR shall have close coordination with other parties who interface to identify existing buried
utilities in the vicinity of the work areas before any work takes place. A side from the General Work
Permit issued by the COMPANY, CONTRACTOR shall obtain Daily Permit to Work from interface
Landowner.
 CONTRACTOR will send the approved procedure prior to commence the work to COMPANY in other
parties who interface.
 Whenever the pipe route preparation activities cross housing driveways, entrance to private properties
or other access road, information shall be given to the owner at least 24 hours and not more than 2 x
24 hours prior to commencement or the work.
 CONTRACTOR shall invite the representatives of all parties who interface with the construction work,
other pipeline operator to witness the construction work process.
 All necessary precautions shall be taken to protect existing pipelines or utilities from damage.

12 PRE-CONSTRUCTION WORK

The construction area along ROW, The contractor will clear that portion of the ROW which is deemed
necessary to accommodate for safe & practical execution for pipeline construction & installation work,
such as for temporary stock pile areas, construction access, working space and parking areas at site.

12.1 Survey

Prior to commencement of the construction activities, in general surveyor shall mark the centerline pipe
and limit boundary brownfield – greenfield (2 meter from edge existing) and any crossing utilities existing
using wooden pegs at 50 m interval.

Care shall be taken to ensure that the centerline and boundary markers are maintained during construction
operations.

Photographic record of ROW before, during and after construction shall also be taken and made available.

12.2 Access

Access to the ROW shall normally be from public roads which intersect the ROW, or other suitable access
points, providing prior consent has been obtained from the relevant authorities, and all necessary permits
have been obtained.
All trees, which are along and outside of the pipeline corridor, which are not to be cleared, shall be clearly
marked. No trees outside the ROW shall be felled.

12.3 Existing Pipeline

The location, alignment and depth-of-cover of the existing underground pipelines and other existing
underground utilities shall be verified via trial hole, necessary precautions to prevent any potential
damages shall be taken care. For instance, steel plate will be installed aboveground

on top of the existing pipeline or utilities in order to minimize external loading from any vehicular crossing
(excavator etc.).

12.4 Existing Waterway

Where grading operations interfere with existing waterway or irrigation supplies which across ROW, care
shall be taken to maintain such services by utilizing temporary measures, such as flume pipes, of
appropriate size to maintain the natural flow. Reroute the waterway if necessary.

13 PROCEDURE FOR ROW CLEARING AND GRADING

Prior to start clearing and grading activities, the following precautions shall be implemented:

a) Ensure that the correct PPE is provided and worn by the workers.
b) Perform Pre-job meeting attended by all the personnel involved in the work.
c) The Supervisor to ensure that all workers have had the correct induction and training.
d) The Supervisor shall clearly explain the work permit and JSA to all personnel involved.
e) The Workers will participate in updating or modifying the JSA to meet actual work conditions.
f) Check the equipment and tools to be used; any damaged equipment and tools shall be tagged repaired
or replaced prior to use.
g) Set-up barrier, signage, and warning sign at working area.
h) Confirm that Liaison Officer have informed local community of the upcoming work.
i) Ensure that the work site is clear of non-authorized personnel.
j) Install temporary erosion control measures, such as sand/soil bags.
k) Install temporary mat (steel plate) if deemed necessary for the equipment to be moved across the
underground existing pipelines or utilities.
l) Pick up all trash (paper, plastic, wire, etc.) and haul off site. Dispose at the identified waste area by
local authorities grass, trees, structure, site debris, etc.
m) Supervisor shall submit progress report or document to QC inspector for further data process.
n) Security patrols will be used to keep the working area free from non-authorized persons and local
community and to provide security for equipment and tools.

13.1 Clearing Works

The clearing & grading works will be performed by the following sequences:
a) In general, site clearing shall be performed & cover throughout the 15 meters width of ROW based
on reference to the installed ROW boundary markers at site. CONTRACTOR shall clear the portion
of the ROW where is deemed required to accommodate for safe & practical execution for pipeline
construction & installation work such as hauling & stringing, fit up & welding, NDT, FJC, lowering,
& backfilling.
b) The existing soil berms which diagonally across the identified working space. This is to prepare flat
ground surface prior to top soil grading and subsequent pipeline installation work as mentioned.
Some berms will not be completely removed in order to be utilized as support for pipe stringing.

c) In principle, any vehicle and machineries shall avoid from crossing on top of the existing. However,
if really required to cross on top of the existing pipeline; the vehicle/machineries shall be limited to
cross by using the provided steel plate protection only, whereby the number of crossing shall be
minimized. Request permit to work from COMPANY is required.
d) Any ROW features such as existing warning signages, ROW boundary markers, pipe.
e) The working area shall be cleared by removing the trees and vegetation as necessarily, including
overhanging branches which may obstruct to future pipeline construction. Any overhanging
branches or structure must be resolved with owner (if any) prior to clearing list of the overhanging
obstacle. After removing/felling the trees and cutting branches off, branches and logs may be placed
at the edge of ROW provided temporarily.
f) Ensure all of the roots of plants raised inside the ROW which interfere the upcoming pipeline
installation work shall be removed during the site clearing activity.
g) Clearing shall be restricted to the approved ROW, and to the extent practical, limited to that
necessary for the safe and efficient operation of equipment.
h) Open burning is not allowed at any time during the construction activity.

13.2 Grading Works

a) In general, the top soil of ROW shall be graded as to prepare flat ground surface for pipeline
installation work. Grading of top soil will be performed using excavator or buldozer.
b) Temporary waterway (small ditch) shall be prepared by at the edge of ROW. This is to limit any
potential of uncontrolled surface water from flowing to outside of ROW. This can be controlled by
preparing cut and leveled slope transversally to the edge of ROW limits. The waterway shall be
drained to nearest existing waterway (ditch, irrigation, etc.).
c) After grade level shall be measured and recorded in order to design depth of trench.

13.3 Disposal of Waste Material and Debris

Trees, rock, stumps, brush and other debris shall not be pushed off the work area. Trees, brush, stumps
and other encumbrances shall be cut flush with the finished grade. Stumps within 4-5 m of the centreline
of pipeline alignment shall be removed by grubbing. All trees cleared shall be felled in a way to ensure
they fall within the working area.

The above works may be performed by excavator or by using hand tools such as axes or machetes, all
felled vegetation will be removed to the temporary disposal area/location.

13.4 Major Site Clearing (except at normal ROW)

Site clearing at the any next to existing the Scrapper Launcher Station (rubber trees), palm trees and other
big trees will be remove usiing buldozer or bucket excavator. If required, remove big trees using chainsaw.

Ensure all of the roots of plants raised inside the ROW which interfere the upcoming pipeline installation
work shall be removed during the site clearing activity.

13.5 Interface

The approved procedure shall be distributed sent prior to commencement of work to all parties involved.
Representatives of all parties who interface with the construction work shall be invited.
All necessary precautions shall be taken in to account/consideration to protect the existing pipeline or
utilities that will be crossed by the new pipeline.
The pre-existing conditions of the road and waterway crossings condition shall be recorded and the
photograph shall be taken as reference for reinstatement.
