Punjab Interschool Women’s Junior Age group Swimming Championship, Lahore

Dear Parent(s),

Your child, Zaynah Murtaza Hassan of Class VIII B has been selected to participate in the Punjab Interschool Women’s
Junior Age Group Swimming Championship to be held on Saturday, 20th April 2024 at the Punjab International Swimming
Complex near Gaddafi Stadium, Lahore.

Please note:

● Parents are requested to submit one copy of their child’s NADRA B – Form, CNIC OR FRC and one recent passport
size photograph to the Admin Office by Friday, 19th April 2024.
● Parents are requested to submit a registration amount of Rs. 2,000/- along with their child’s ID documents on
Friday, 19th April 2024.
● Students will be required to bring their original B Form to show at the venue.
● Participants must arrange their own transport and report to the venue at 8:30 a.m. sharp and be picked up when
the event concludes. Parents will be given a schedule of the event at the venue and may have to wait depending
on the timings of the championship.
● Participants must be dressed in complete School PE Uniform and bring the Swimming Kit, water bottles and a
packed lunch with them.
● Participants must adhere to the Learning Alliance Code of Conduct at all times.
● Participants will be chaperoned by the swimming coach Ms. Farhat, Ms. Aymen Rashid and Sir Khurram Aftab.

If you permit your child to participate in the contest, kindly sign the permission slip below and return it, along with all
other required documentation to the Admin Office no later than Friday, 19th April 2024.

Permission Slip

“Punjab Interschool Women’s Junior Age Group Swimming Championship” to be held at Punjab International Swimming
Complex near Gaddafi Stadium, Lahore on Saturday, 20th April 2024. I understand that the host is solely responsible for the
security of their premises for the duration of the event, and I am allowing my child to attend this event at my own responsibility,
and under the full knowledge that LEARNING ALLIANCE staff will only be chaperoning my child at the venue.
