SPECIAL POWER OF ATTORNEY

That I, JUAN DELA CRUZ, of legal age, Filipino Citizen, with postal address at Brgy.
Uno, Nueva Caceres, Camarines Sur, by these presents do hereby NAME,
CONSTITUTE AND APPOINT ______________________, Filipino citizen with
residence and postal address at _______________________, to be my true and
lawful Attorney-in-Fact, for me and in my name, place and stead to do and perform all of
any of the following:

1. To process the all that is necessary and indispensable under the law for the
registration of my bed and breakfast business located at Brgy. Uno, Nueva
Caceres, Camarines Sur, which include, but are not limited, the following:
a. To register a business name at the Department of Trade and Industry;
b. To register the business at the Bureau of Internal Revenue;
c. To register the business and obtain a permit at the Mayor’s Office.

2. To sign acknowledgment receipt or any other document of whatever nature or

kind of which needed my consent or personal authority, or make payments which
may be necessary or required in performing and consummating the aforesaid
acts;

3. To do any other act as may be necessary in performing and consummating the

aforesaid acts.

HEREBY GIVING AND GRANTING unto my said attorney full powers and authority to
do and perform all or any acts requisite and/or necessary to carry into effect the
foregoing authorities, as fully to all intents and purpose as I might or could
lawfully do, if personally present and acting in person, and HEREBY
RATIFYING AND CONFIRMING all that my said ATTORNEY-IN-FACT may also do
or cause to be done under and by virtue of these presents.
