Has consideration been made to method of attaching the Injured person? (Detail below emergecy instruction)

 In case of one heavy equipment (Crane 220T) was get oil leakage on hydraulic part when lifting activity on
BOILER area happened, report to Supervisor, Safety personnel or ERT in charge, do not panic
 Stop related activity
 Give detail report information about Crane 220T condition including part of hydraulic that was damage
 Make sure material already release on below elevation when operator know that the hydraulic was leakage
 KOIN safety personnel report to ERT KOIN commander
 ERT KOIN commander will forward this information to first aider and evacuate team, wheatear any victim from
workers, ERT KOIN Commander also ask to foreman cleaning bring equipment and tools to directly toward to
scene and bring the cleaning team
 First aider and evacuate team observer arrived at the scene, soon they do observer to Crane 220T and around
environment at boiler ground, after confirm that no victim in this accident, first aider report to ERT KOIN
Commander.
 Directly by radio, KOIN ERT commander makes sure to foreman cleaning that already stand by on scene. KOIN
ERT commander by radio inform to evacuate team to install hard barricade around 220T and standby to monitor
the spill. Cleaning team give a sand around the leakage area the purpose is to absorb the oil with the sand after
that cleaning team preparing sack, team cleaning load the sand to sack and preparing light vehicle to transfer
sack to B3 storage.
 After all finish team cleaning continue to clean leakage with spray with water and made clean leakage area.
After all finish evacuate team report to ERT Commander that oil spill already handle with safe, hard barricade
can be open again, lifting activity can continue again.
