RESEARCH QUESTIONNAIRE

Note: The researchers of Grade 11-STEM Escoda created this survey questionnaire to collect
the needed information from the selected students of Sto. Domingo National High School. This
study is entitled “Assessing Students’ Earthquake-related Behaviors and Skills Preparedness
Level: DIY Low-cost Earthquake Alarm System Development”. The gathered information will be
kept confidential and the respondents will not be forced to answer the given questions.

Direction: Kindly answer the following questions completely and honestly. Please put a check
