At the end of this learning activity, the FS students should be able to:
1. identify the school forms, its purpose and uses;
2. download and fill-out sample DepEd school forms; and
3. do sample computation of grades using the DepEd assessment criteria.

The K-12 Grading System, Electronic Class Record Templates, and DepEd Forms
. The K to 12 Basic Education Program uses standards- and competency-based grading system. When we say grading
system we mean a system that educators use in a particular institution to evaluate each individual student’s performance and
learning. Grading can include letter grades, percentages and even a simple pass/fail. Grades can be attached to physical activities
like writing assignments, lab work, projects, reports and tests. In DepEd, all grades are based on the weighted raw scores of the
learners summative assessments. These summative assessment can be classified into three components, namely, written works
(WW), performance task (PT), and quarterly assessment These three will be the bases for grading. The nature of the learning area defines
the way these three components are assessed.
For Kindergarten, checklist and anecdotal records are used instead of numerical grades. These are based on learning
standards found in the Kindergarten curriculum guide. For grades 1 to 12, numerical grades are used. The minimum grade
needed to pass a specific learner’s area is 60, which is transmuted to 75 in the report card. The lowest rank mark that can appear
on the report card is 60 for quarterly grades and final grades.
Teachers in the Basic Education implementing the K to 12 program uses electronic class record in excel format to facilitate
high level of accuracy in the computation of the learners grade. Each learning area has its specific e-class record. Aside from these,
different DepEd forms are accomplished at varied time within a school year.
In this activity, FS 2 students will be exposed to various DepEd forms and e-class records. They will be given
opportunities to examine and accomplish different forms, compute quarterly grades and final grades, and accomplish e- class records
aligned with their specialization.

is discussed. The grading scale its corresponding descriptors, are in table 3.

Answer: Learners A failed to get the passing grade with the initial grade of 54.78 then transmuted grade with 73. The components that need to improve

1. Which is easier to do, manually compute the grade of the students or the use of e-class record? Why?
Answers:
For me, it is easy to compute the grade of leaners with the use of e-class record because it is very organized and can edit any errors. E-
class record systems have built-in formulas and algorithms that calculate the grades automatically based on pre- defined grading criteria.
This eliminates the need for manual calculations, reducing the chances of errors and saving time. E-class record systems provide a centralized
database where all student data and grades are stored. This makes it easy to access and update information, as well as generate reports or
transcripts whenever needed. The use of e-class records offers convenience, accuracy, and efficiency in managing and computing student grades,
making it a preferred choice over manual calculations.

2. Is it important to employ a systematic approach in computing the grades of students? Justify your answer.
Answers:
Yes, it is important to employ a systematic approach in computing the grades of students. A systematic approach ensures consistency in
grading across different teachers, subjects, and semesters. It eliminates any bias or subjectivity in grading, ensuring that all students are evaluated
fairly and accurately. A systematic approach provides transparency in grading, making it clear how grades are calculated and enabling students to
understand how their performance is being assessed. This transparency helps build trust and confidence in the grading process. Employing a
systematic approach in computing grades promotes fairness, consistency, transparency, reliability, and efficiency in the grading process,
benefiting both students and teachers alike.

3. Why is there a need to accomplish the different school forms?


Answers:
School forms help in organizing and maintaining accurate records of students, teachers, and other personnel. These records are
crucial for administrative tasks such as enrollment, tracking attendance, planning schedules, and assessing student progress. School forms often
serve as a means to comply with various regulations imposed by educational authorities or government bodies. These regulations may

4. When are the different school forms need to be accomplished? Why?


Answers:
The different school forms need to accomplished by the beginning of the school year, end of school year, sometimes
quarterly to make sure the quality and consistency of the learner’s information.

5. What will happen if the school forms are not accomplished on the specified schedule? Answers:
If the school forms are not accomplished on the specified schedule, it can lead to various consequences. Not completing the forms
on can result in a delay in processing your application or admission. This means you may not get timely notifications or updates about your
enrollment status or any other important information. To avoid these consequences, it is advisable to complete the school forms within the

Answer the following questions:

1. If you are the teacher, what approach will you use to systematically compute the grades of you students?
Why do you prefer that approach.
Answers:
Weighted Average Method: This method involves assigning a specific weight or percentage to each assessment category (such
as quizzes, tests, assignments) based on its importance in measuring student learning. The grades earned in each category are multiplied
by their respective weights, and then all the weighted grades are added together to calculate the final grade.
For example, if quizzes have a weight of 30%, tests have a weight of 40%, and assignments have a weight of 30%, you
would multiply the average quiz grade by 0.30, the average test grade by 0.40, and the average assignment grade by 0.30.
Finally, you would sum up the weighted grades to determine the final grade.
Ultimately, the choice of grading approach depends on the teacher's preference and the nature of the course and assessments.
It's essential to maintain transparency and communicate the grading method to students at the beginning of the course to ensure clarity
and fairness.

2. Which of the different school forms is the easiest to accomplish? Why?


Answers:
The easiest School form to accomplish is the sf2 because it is just the list of the learner's daily attendance. Just check if who
are the leaners attending classes.

3. Which of the different school forms is the most difficult to accomplish? What will you do to lessen the
difficulty encountered in accomplishing this form?
Answers:
For me, the SF9 is the most difficult to accomplish. To lessen the difficulty encountered in accomplishing this form begin the
application process well in advance to ensure ample time for gathering necessary documents, completing essays, and obtaining
recommendations. This will help avoid the stress of rushing through the form. If available, seek guidance from a school counselor,
college advisor, or mentors who can provide assistance throughout the process. They can provide valuable insights and help navigate any
potential challenges. Remember, it is important to approach the application process with patience and give it the time and attention it
