At the end of this learning activity, the FS students should be able to:
1. identify the school forms, its purpose and uses;
2. download and fill-out sample DepEd school forms; and
3. do sample computation of grades using the DepEd assessment criteria.

The K-12 Grading System, Electronic Class Record Templates, and DepEd Forms
. The K to 12 Basic Education Program uses standards- and competency-based grading system. When we say
grading system we mean a system that educators use in a particular institution to evaluate each individual student’s
performance and learning. Grading can include letter grades, percentages and even a simple pass/fail. Grades can be
attached to physical activities like writing assignments, lab work, projects, reports and tests. In DepEd, all grades are
based on the weighted raw scores of the learners summative assessments. These summative assessment can be
classified into three components, namely, written works (WW), performance task (PT), and quarterly assessment These
three will be the bases for grading. The nature of the learning area defines the way these three components are assessed.
For Kindergarten, checklist and anecdotal records are used instead of numerical grades. These are based on
learning standards found in the Kindergarten curriculum guide. For grades 1 to 12, numerical grades are used. The
minimum grade needed to pass a specific learner’s area is 60, which is transmuted to 75 in the report card. The lowest
rank mark that can appear on the report card is 60 for quarterly grades and final grades.
Teachers in the Basic Education implementing the K to 12 program uses electronic class record in excel format to
facilitate high level of accuracy in the computation of the learners grade. Each learning area has its specific e-class record.
Aside from these, different DepEd forms are accomplished at varied time within a school year.
In this activity, FS 2 students will be exposed to various DepEd forms and e-class records. They will be given
opportunities to examine and accomplish different forms, compute quarterly grades and final grades, and accomplish e-
class records aligned with their specialization.

The summary of learners progress is shown quarterly to parents and guardians through a parent-teacher conference, in which
the report card is discussed. The grading scale its corresponding descriptors, are in table 3.

Briefly describe the approach you used to systematically compute the grade of the learners.

The approach I used to get a comprehensible grade of the learners in order to know how much and how well they’re learning
was according to the weights of components of DepEd learning area grade 1-10 may carry the different weights in order to determine
final grade. This specific approach provides an accurate results of the learners to diagnose their weakness according to their
performance and helping them understand their strengths and areas for improvement.

Answer the following questions:

1. Which is easier to do, manually compute the grade of the students or the use of e-class record? Why?

Based on my experience while computing grades I can assure that using E-Class record was much easier than manually compute
because nowadays instructors may need to adapt technology to save their precious time and get an accurate result of grades while
manually calculating grades for large classes or complex grading systems can be time-consuming and prone to errors.

2. Is it important to employ a systematic approach in computing the grades of students? Justify your answer.

In my perspective it’s very important to employ a systematic approach in computing the grades of students because systematic
approach ensures that all students are evaluated fairly using the same criteria and methods, by employing systematic approach
in computing grades promotes fairness, accuracy and efficiency in educational assessment process to align the principles of
education quality helps both students and teachers to supports continuous improvement of teaching and learning.

3. Why is there a need to accomplish the different school forms?


School forms is administrative requirement in educational system it is important to accomplish to maintain an organize and
comprehensive records of students and school related information to provide a means to track and assess the performance,
progress, academic achievements and relevant data thought out their educational journey. The completion of various school
forms is crucial for keeping correct records, guaranteeing accountability, adhering to regulations, evaluating academic
performance, properly allocating resources, and promoting communication within the educational system.

4. When are the different school forms need to be accomplished? Why?


School Form 1(School Register) need to be accomplished by beginning of the school year and of the school year. School Form
2 (Learners Daily Attendance) must be monitored. School Form 3 (Books issued Return) by beginning of the school year and
end of school year. School Form 4 (Summary Enrollment and Movements of Learners) Monthly. School Form 5 (Report on
Promotion) End of school year. School Form 6 (Summary Report on Promotion) End of school year. School Form 7 (Inventory
of School Personnel) by beginning of the school year and end of school year. School Form 8 (Learner Basic Health and Nutrition
Profile) By beginning of the school year and end of the school year. School Form 9 (Learner Progress Report Card) Quarterly.
School Form 10 (Learners Permanent Academic Report) End of School Year. These different school form has important specific
purpose of entire school in order to completely organize the event and activities timely completion of these forms is crucial for
maintaining accurate records, ensuring compliance with regulations, and supporting student well-being and academic progress.

5. What will happen if the school forms are not accomplished on the specified schedule?
If the school forms are not accomplished it can cause a conflict situations of activities, events and disorganized forms of entire
school.

Answer the following questions:

1. If you are the teacher, what approach will you use to systematically compute the grades of you students? Why do you prefer
that approach.

As a future educator I prefer E-class to compute the grades of students because it’s much easier to get an accurate result of
students and E-class have become an integral part of modern education, this platform enhance accessibility, flexibility,
engagement and effectiveness of teaching and learning it has a potential to education adapt technology trends and make it
easier to individuals to engage life-long learning and professional development to widen their skills and knowledge.

2. Which of the different school forms is the easiest to accomplish? Why?


Based on my experience School form 2 (Learner’s Daily Attendance) is easiest to complete because it’s accessible and
available to download in any DepEd website and it easy to use to monitor the pupil’s attendance by checking on it.

3. Which of the different school forms is the most difficult to accomplish? What will you do to lessen the difficulty encountered in
accomplishing this form? Based on my experience the most difficult school form that I accomplished is school form 7
because it’s the list of personnel profile and official duties of teachers that contains their respective assignment and
responsibilities inside the school so, this school form contains a data privacy and security matters.
