EXCERPTS FROM THE FOCUSED GROUP DISCUSSION (FGD) OF

JUNIOR HIGH SCHOOL TEACHERS HELD ON APRIL 13 (3:45 PM – 4:30 PM) AT
THE GUIDANCE OFFICE.

1. School Learning Action Cell (SLAC) Form 4

As part of the prescribed post-process in conducting SLAC session, teachers were

reoriented in in accomplishing the SLAC Form 4 that serves as feedback form of the
teacher-participants’ who attended the session. Furthermore, the form also serves as an
avenue for the teachers to reflect their learnings and suggestions.

2. Individual Performance & Competency Rating Form (IPCRF) 2020-2021 Mid-Year

Review

In realization of the mid-year review of the IPCR 2020-2021, teachers were advised
to prepare their portfolios for the first half of the school year. Moreover, each of them was
scheduled for classroom observation post-conferences.

3. Compilation of Previous School Forms and Anecdotal Records

To guarantee a smooth transition of records from the previous school year to the
present, all advisers last year were instructed to surrender the SF 10 and anecdotal records
of their respective previous class advisories to Mrs. Jovelyn Agsalud, Teacher III. The latter
will, in turn, forward the school forms to the present grade level advisers.

4. Homeroom Guidance Program (HGP)

To make sure that the school is in consistent compliant to the new HGP of the
department, class advisers were reminded to download and print the HPG modules and
accomplish all the required the forms annexed to the HPG implemntaion memorandum.
5. Updated School Forms

Before the meeting was adjourned, Mr. Romuald A. Abulencia, Teacher III, discussed
the content of Deped Memorandum No. 14, s. 2021, Interim Guidelines on the Preparation,
Submission and Checking of School Forms for the School Year 2020-2021. The focused of
the presentation was on how to convert the participation of both online and modular students
into one-day or one-week attendance count since there is no face-to-face classes due to the
pandemic. Another note made was the additional table to be printed onto the SF 9 or the
report card of the students. The table shall reflect the mode of learning the students enrolled
have enrolled in.

The SLAC was adjourned at 4:30 PM.
