VENUE DESCRIPTION

In the hubbub of Tandang Sora in Quezon City, the Santuario de San Vicente de Paul has
hosted a lot of important events in people’s lives, such as wedding ceremonies and
baptisms. The Atrium, an close-air pavilion can accommodate up to 200 pax. The
Verandas bears the professional touch of the same people who worked on Jardin de
Miramar in
Antipolo, so one can expect the same level of beauty and quality in this venue.
The Verandas is yet another way the Santuario de San Vicente de Paul brings people
together under a happy community.

I. SPECIAL PACKAGE COST
In appreciation of your interest, we are offering hereunder a special package
incorporating professional services for event management, hosting, staging the
event, venue, supplies, food and drinks, lights and sounds.
The special package cost is PHP 74,415.50

III. CANCELLATION POLICY


Cancellation made after payment option date shall not be entitled to a refund.

We shall require 50% downpayment upon confirmation. Full payment will be required on the day of
the said event itself.

TERMS AND CONDITIONS


CARPE OMNIA EVENTS MANAGEMENT INC. will provide full professional
service including but not to limited to event managing, hosting,
staging, directorship, and will provide the necessary supplies for
KRISTOFF DALE’S BAPTISM which will be held on OCTOBER 7, 2018
(SUNDAY), from the hours of 11:00 AM- 2:00 PM ST. PETER PARISH
CHURCH (BAPTISM CEREMONY)
VERANDAS DE SAN VICENTE DE PAUL (RECEPTION),
QUEZON CITY PHILIPPINES will pay the total amount of PHP 74,415.50 for
the entire service provided. One half of the total expenses PHP
38,208.25 is to be paid upon completion of this contract as a deposit
to hold the date, (deposit is not refundable) and the balance of the
total amount to be paid on the date of the event.
CARPE OMNIA EVENTS MANAGEMENT INC. will furnish labor and
equipment, materials, including set-up and breakdown.

PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK
In consideration of the acceptance and participation in the services of CARPE OMNIA Event Management, their agents, owners, officers,
volunteers, participants, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively as CARPE OMNIA Event
Management), I hereby release, indemnify, hold harmless, and discharge CARPE OMNIA Event Management from any all liability for injury or harm that I or
anyone might sustain and I hereby release, hold harmless and discharge CARPE OMNIA Event Management from any all claims, demands, or causes of
action, which are in any away connected to my participation in this activity or use of CARPE OMNIA Event Management equipment to the extent allowable
under State and Federal Laws.

I. I acknowledge that my participation entails known and unanticipated risks that could result in injury or damage to myself, property, or to third parties. I
understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity.

THE RISKS INCLUDE, AMONG OTHER THINGS: FALLING OFF THE STAGE RESULTING IN MUSCOLO SKELETAL INJURIES INCLUDING HEAD, NECK, BACK, HAND, AND
ARM INJURIES.

Furthermore, CARPE OMNIA Event Management employees have difficult jobs to perform. They seek safety, but they are not infallible. They might be ungive
incomplete warnings or instructions, and the equipment being used might malfunction.

2.I expressly agree and promise to accept and assume all the risks existing in this activity. My participation in this activity is purely voluntary, and I elect to
participate in spite of the risks.

3.I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless CARPE OMNIA’s Event aware of a participant’s fitness or abilities.
They may Management from any and ALL claims, demands, or causes of action, which are in any way connected with my participation in this activity or
may use of CARPE OMNIA’s Event Management equipment or facilities, including any such claims which allege negligent acts or emissions of CARPE
OMNIA’s Event Management

Should CARPE OMNIA’s Event Management or anyone acting on their behalf, be required to incur attorney’s fees and costs to enforce this agreements, I
agree to indemnify and hold them harmless for all such fees and costs.

5.I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such
injury or damage myself. I further certify that I am willing to assume the risk of any medical or physical condition I may have.

6.I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect.

By signing this agreement, I acknowledge that if anyone is hurt or property is damaged during my participation in this activity, I may be found by a court or
law to have waived my right to maintain a lawsuit against on the basis of any claim from which I have released them herein
