the following Schol Forms; however
the following are suggested points
or considerations with regard to
accomplishingthe following School
Forms:

1. SF 1
 Learners’ data are encoded
thru LIS
 Forms can be generated thru
LIS
 No adjustment is needed

2. SF 2 – Learners’ Daily Attendance
Possible bases of Learner’s Attendance :

 Accomplished Activities and answer sheets ( If a learner did

not answer the activities in the module and passes a blank
answer sheet – means absent)

 Retireval and distribution of modules ( Modules claimed

and returned on time means present ; Modules claimed and
returned beyond schedule means late; Parents and gurdian
with valid excuses for late/unclaimed/unreturned modules
means excuse;
 Online conferences

› The adviser will pass and
submit soft copies of SF 2
for consolidation in the SF 4
in the school google drive
for easy retrieval .

3. SF 3 – Books Issued and Returned
 Instead of books, the Adviser will record the
modules distributed and retrieved;
 This will be accomplished weekly by the
advisers;
 The Advisers will pass the soft copy of SF 3 thru
google drive to the School Custodian for record
keeping;
 Only Official modules (from CO or DO) shall be
reflected in the SF 3;
