• Monday is CHECK DAY for the Learning Plans for next week. This is to be initially Checked by the Following “Hari-hari”
1. Kindergarten to Grade Three will be Checked by Sir Troy Villorente
2. Grades Four to Six will be Checked by sir Leonard Icao
3. Grade Seven to Nine will be Checked by Sir Ramonito Villela
4. Grade Ten to 12 will be Checked by Maam Bless Palomar
5. Please used Friction Pen for the dates to be easily removed once compiled. For easy recycling next School Year
• All Learning Plans for next week must Be checked every Monday to be signed by the School Head after Initial Checking of the
Asst. School Head or Dept Heads assigned.
• Please Open the Backdoor near the teachers table for Easy Access in your classroom to minimized disruption of classes every
checking and Monitoring period. For Teachers with no standard classroom, please hand over the prepared LPs to the school
head

Physical Facilities and ICT Equipment and Utilization/Borrowing System
• All School Equipment's must be tracked
• In-charge/Coordinator will be given logbook this is to track the Name of Responsible
Person, Time and Date Borrowed/Returned, Purpose and Signature.
• Meanwhile, Laptops, TVs and other equipment that were issued to the teachers or school
head must have PTR(Property Transfer Report). This is to ensure the full responsibility of
the school equipment. However, if to be used in an exigency of time and urgent reports
to be accomplished by other teachers in their school or district coordinatorships. They
may ask to borrow the equipment in a certain period of time with proper tracking
procedure.
• Allocation of the Provision of Basic TVs will be included in the NEXT AIP for FISCAL YEAR
2023
• Return all Functional and Non-Functional Equipment for Inventory Purposes assigned on
a target date.

• All School Supplies must be tracked

• In-charge/Coordinator will be given logbook track the Name of Teacher,Item, Quantity,


Signature.

• In case A teacher needs special materials to be used in classroom structuring or other


related classroom learning materials, a teacher may write a request a week before to
give time for the procurement of the item. This will be addressed to the property
custodian to be approved by the school head.
