WHAT HAPPENED: Bulacan plant demolition. On Feb 19 th, Gerald Obligado, 34 y.o., married and four children, contractor safety officer, lost his life on site
after after falling from the top of a 13 meter staircase during demolition activities. At around 9:30 am, Gerald and one of his colleagues were in the process of
bringing down the spiral staircase in a demolition works at the old cement line. The lower portion had already been cut off by the cutter under the supervision of
Gerald. They both proceeded to the top of the spiral staircase through alternative access routes to cut the remaining terminal joints of the staircase from the building.
Gerald’s colleague was positioned at the concrete platform of the storage hall, wearing a full body harness which was anchored to a rigid structure behind him.
Gerald stepped on the top most step of the spiral staircase without wearing any fall protection. Aware of the risk, the cutter cautioned Gerald not to stand at the
top of the staircase. According to him, Gerald looked back at him and ignored his warning. In about 5 minutes the entire staircase fell forward together with Gerald,
crashing down to the ground. Gerald was immediately brought to the hospital but lost his life the next day after.

2011 – Ecuador – Fall from height– Opening / weak structure

WHAT HAPPENED: On June 28, 2011, at approximately 17h30 and employee of a third party service provider, suffered a fall from about 20m high, resulting in his
death. The victim was working as a helper carrying pipes for a compressed air system along a conveyor belt structure (100m long with 15% slope) under construction.
According to an eyewitness right after walking up, for resting purposes the victim sat on an intermediate handrail’s tube and fell backwards through an opening (81 cm)
between the bottom and top rail tubes. The person was wearing his full personal protective equipment, including harnesses and life lines corresponding to the task that was
being performed and anchoring points were available at this point .

WHAT HAPPENED: Gongxian site. On October 19 th, Ni JianHua, aged 28 , married, and leader of a contractor’s erection team contractor, died on site. Ni
JianHua, was working with his team on the erection of the cement mill elevator and he was standing on 3 wooden plates installed on the open top of the
elevator. At certain moment, for an unknown reason, the temporary support wooden structure above him collapsed. Ni JianHua was struck on the head by a beam,
which caused him to fall to the bottom of the elevator, 40 meters below. Ni JianHua was declared dead at his arrival in the hospital.

WHAT HAPPENED: Megdigia L2 site. On Sep 29 th, Mr. Gench Dzevat, aged 49. married and 2 children, worker of a contractor erection company, died after
he fell from a ladder while performing preassembly works. Gench and two of his colleagues were adjusting the inlet part on the future stack of Medgidia line 2. Gench
was standing on the 3rd rung (0,9 m) of a ladder, hold by a co-worker. He was installing a pressure screw to reduce the gap between parts in order to allow welding.
Suddenly he lost his balance and fell down. It is believed that in the process of adjusting the screw fitting, the anchor of the chain block which was maintaining
both parts in position failed, leading the chain block to fall and pushed Gench out of balance. He suffered multiple skull fractures and died two days later in the
hospital.

WHAT HAPPENED: Chanda site. On 19th September 2010, an employee of a third party service provider was fatally injured as a result of falling approximately 13.5
meters from a scaffold onto a concrete floor below. The deceased was assigned the task of installing floor gratings in the upper level of the bag house (air slide platform). At
about 09:00 h the deceased has started climbing through the scaffolding, he has reached the temporary platform. It appears the deceased has reached over to grab the
guide rope which is used to pull up the floor gratings, when he has lost his balance and fallen to the concrete floor below. The deceased sustained head injuries and was
immediately shifted to a hospital where he succumbed to his injuries .

WHAT HAPPENED: Garadagh site. On 13 December 2008, three employees of a third party service provider carried out painting works on the new Main
Power Substation construction site. They have been instructed to dismantle a mobile scaffold of the height of 6.4 m at one working place and to re-erect it at the
other. While the supervisor was absent, it seems that the workers had decided to move the scaffold as a whole to the next workplace by leaving the area
through one gate and to re-enter the construction site from an other gate, using a public road. While doing so, the scaffold made contact with a 35 kV state
owned power line leading from Puta village to Sangachal village. As a consequence, three workers were electrocuted, two of them died immediately, one was
seriously injured.

WHAT HAPPENED: On 27th December 2008, in Bhatapara, India, an employee of a third party service provider was fatally injured as a result of falling approximately
9 meters inside the plant’s calciner. The victim was trying to remove an angle bar (angle iron) welded to the inside wall of the calciner by using an oxyacetylene torch.
Preliminary investigations indicate that the victim was wearing a safety harness fitted with a double lanyard in which he had used one of the sections of the angle bar as an
anchor point. The victim secured himself to the anchor point (i.e. the angle bar) by tying the lanyards in a knot and placed them over the top of the angle bar. It appears that, as
the victim lost his balance, the lanyards slept over the open end of the angle bar, thus allowing the victim to fall to the lower level of the calciner.

WHAT HAPPENED: Fumin project (Yunnan – China). On October 13, Zhang TaiJun, aged, hired 3 days before the accident, was killed in a fall from a
height. Two contractors were working on reinforcing a structural wall. At about 6.30 p.m., one of them decided to hurry down from the work platform to get to
the rest room as he felt sick. Instead of climbing up the normal way to reach the road which was on top of the wall, he started to climb directly down the
scaffolding at a height of 14 meters. After waiting quite a long time and not seeing his colleague return, the second man climbed down, and found Zhang
TaiJun lying on the ground at the foot of the scaffolding. The victim was immediately transported to the regional hospital and then to Kuming hosptal, where he
died.

WHAT HAPPENED: Sonadih L2 site. On April 10, Yash Kumar, age 23, who was employed by a mechanical erection contractor, fell 14 meters to his
death. Yash Kumar was working the night shift along with two welders employed by the same contractor, and was stationed on a working platform at a height of
71 meters inside the cyclone under construction. At about midnight, Yash Kumar fell from the platform inside the cyclone. His fall was broken 14 meters below
(at a height of 57 meters). He had been wearing all his PPE, including a double rope full body safety harness, but the safety harness was apparently not
anchored. He had been assisting welding teams for five months, and had attended all the safety training sessions available on the site..

WHAT HAPPENED: Lichtenburg L2. On April 10 at 10 a.m., Jiang Min, age 26, married and father of 3, and Zheng Jiechen, age 24, engaged, were victims of a fatal
accident when a set of vertical bars from the clinker silo on which they were working fell on them. Two other workers, age 31 and 20, were seriously injured, and seven more
were hurt to varying degrees. They were all employed by our contractor. The accident occurred while 41 workers were erecting the reinforcement of the clinker silo wall. They
were fixing the vertical rebar. They had completed 80% of the work when the vertical bars suddenly collapsed with a domino effect all round the silo. Seven members
of the team were trapped in the reinforcement cage. Jiang Min died while being transported to hospital. Zheng Jiechen lost his life the following night while under intensive
care at the hospital.

WHAT HAPPENED: Garadagh site. In the morning of Monday, 15 November 2010, the project main turnkey contractor was working at their Site Fabrication Workshop
area. One Contractor’s worker did tack welding on the bottom of a steel plate, which was lifted and vertically held in two bulldog clamps by the boom of a mobile crane. The
vertical hanging steel plate had to be connected right angled with a horizontal steel plate laying on the ground by welding. At around 11:30h a.m. the accident occurred. The
vertical steel plate slipped out of the bulldog clamps and tilt down towards the worker who was welding at the bottom . The worker tried to escape but he was caught
by the overturning plate. His body was pushed down and legs and abdomen were hit by the heavy steel plate. The Contractor’s Site Medical Service was immediately called,
rescued the worker and brought him with the Site ambulance car to the City Hospital. The worker had heavy internal injuries. He passed away at the City Hospital at around
17:24h during the emergency operation.
LIFTING PLAN shall be implemented for lifting operations and it shall include a detailed task analysis considering common hazards such as wind speed, obstructions from
other structures, people working below a load being lifted.

WHAT HAPPENED: Lightenburg L2. On Sep 15, 2008 at 13:45 hours, Wanquan Jin, aged 40, married and father of 2, CBMI refractory worker was
severely injured when his leg was pinned under a partially completed side gate valve when the sling that was used to move it suddenly snapped. The accident
crushed his left leg and caused severe internal injuries to his pelvic area. After undergoing emergency operation at the local hospital, he passed away the day
after. After having secured the formwork to complete the refractory to an air duct slide gate valve, a foreman and 4 workers were trying to turn the slide gate
valve over. Instead of calling for a qualified rigger, the foremen of the refractory team decided to do the rigging himself without checking if the 1.5t
sling that was dangling from the gantry crane was suitable. The estimated weight of the partially completed slide gate valve is about 7 tons. As the gate
valve was not aligned properly, the foreman tried to lift it into position. The sling snapped, causing the gate valve to fall back. Wanquan and another worker
were standing about 1.5 m from the slide gate valve. The other worker managed to step back on time. Wanquan’s movement was too slow and his left leg was
pinned under the gate valve .
RA + PLAN. Ensure that all the personnel involved in the operation are aware of the way to perform the activity and the risks associated to the task and
ensure that appropriate equipment, PPE and collective protection is in place and ready to use. Ensure the appropriate rigging equipment is available and in a
.
serviceable condition, prior to slinging the loads. This includes the provision of proper lifting chains, slings, lifting frames and/or cradles and FIXATION OF
LOADS. .
LIFTING PLAN shall be implemented for lifting operations and it shall include a detailed task analysis considering common hazards such as wind speed,
obstructions from other structures, people working below a load being lifted

WHAT HAPPENED: La Rochelle site. On January 24th, at 19:20 hrs an employee of a third party service provider died after falling 28 meters. The contractor was in
charge of building a silo, whose inner cone consists of 32 prefabricated concrete elements which are installed one after the other. The victim had climbed to the top of a
prefabricated element to finalize the adjustment of the adjacent element using a laser distance meter. He had secured himself by attaching his harness to a metal insert
embedded in the prefabricated element. This reinforced concrete element, weighing 13 tonnes, had been installed and adjusted to its final position 4 days earlier, on the
inner support of the silo provided for that purpose, and held inclined at an angle of 60° (like the other 25 identical elements already installed and adjusted).This inclination at 60°
was maintained by an adjustable tie rod connected to two anchor points (one on the inside wall of the silo; the other on a mild steel lifting bail embedded in the prefabricated
element during manufacture). The mild steel bail broke for reasons as yet underdetermined, causing the prefabricated element to topple and the victim to fall .

WHAT HAPPENED: Hima Rwenzori. On February 7, Stephen Shauri, age 45, a truck driver for the Civicon Transport Company, was crushed when a tank
fell from his truck, killing him on the spot. At about 8.00 a.m., Stephen parked his truck in the CBMI storage and unloading area. The truck’s cargo consisted of 9
meter long steel structures and two tanks seated on tires on the platform at the front of the trailer. While the CBMI unloading team (a crane driver, a rigger and two
local helpers) were starting to prepare to lift the steel structures, Stephen and his turn boy released the latches on both tanks without instruction from CBMI. Loaded
on tires, the cargo was unstable. One of the tanks fell on Stephen, who was killed on the spot. The unloading operation was under CBMI’s responsibility. The
driver and his turn boy were not given clear instructions (not unlashing the cargo before rigging the load and staying out of the lifting area).. There was no CBMI
safety officer controlling the operation.

WHAT HAPPENED: Rauri site. On 26th April at approximately 15:30 hrs an employee of a third party service provider was fatally injured as a result of being run over
by a truck trailer. The incident occurred when the victim (welders’ assistant) and his coworker (fabricator welder) had taken up position underneath the truck trailer to seek
shelter from the sun whilst they were taking a break from their task of cutting scrap steel. The driver started the truck and drove the truck forward unaware of the two workers
situated underneath his truck. The victim was positioned on the inside section of the trailer next to the rear wheels of the truck as it drove forward. The victim’s coworker was
able to escape from underneath the truck. Preliminary information indicates the victim and his coworker may have been distracted by listening to music on a mobile
phone when the truck started its engine which may have account for the delayed reaction when the truck engine started prior to the truck moving forward.

being crushed underneath a tilt trailer and excavated soil. The incident occurred when a contracted tractor was attempting to tip soil from its trailer. As the trailer
was being raised to tip its load (excavated soil) the trailers wheels sank into the ground (soft soil). This caused the trailer to become unbalanced resulting in
the trailer toppling over. At the time of the incident the victim was standing near the trailer and as the trailer toppled over it struck the victim and covered him in
the soil which was being dumped.

WHAT HAPPENED: Fumin project – quarry. On 29th April , Mr Shi Rui, aged 29, married with two children, haul truck driver for a local subcontractor was run over by his
own truck. He was working in the overburden removal for a new quarry area At certain moment Mr Shi’s truck tripped and he asked another haul truck driver nearby to help restart
it by pushing his truck in reverse. The two drivers agreed on this approach, and agreed to start reversing after hearing a horn from the broken down truck. Mr. Shi’s truck manual
startup switch is located under the truck near the right front wheel where the other driver helping him could not see him from his cabin. The helping second driver heard a horn and
thought it was from Mr. Shi’s truck. Unfortunately, there were other trucks working in the area, it is not exactly clear whose horn Mr. Qi heard. After hearing a horn, The
helping driver pushed his truck while Mr. Shi was still squatting at the right front wheel. As result, the broken down truck was pushed forward 2 meters and Mr. Shi was run
over. Immediately, onsite supervisor called the ambulance and took Mr. Shi to the plant entrance to wait for an ambulance. Mr Shi sucumbed in the hospital where he was
transferred.

WHAT HAPPENED: Dujiangyan. On July 16th, Xu Ying Zhi, aged 32 (married with 1 daughter), worker for a subcontractor as quality manager and with 14
years of experience, got run over by an excavator while he was conducting maintenance works on a temporary access road under construction. Xu Ying Zhi was
acting at the same time as the spotter and the works supervisor. Mr Xu and his project manager were standing next to an excavator which was operating in the
area while they were discussing and looking to other direction. At certain moment, the excavator passed near them and touched the soil around them and
provoking they to loose stability making that the excavator passed over Mr Xu before the excavator operator got alerted by the project manager. The
excavator was operated by a 21 y.o. operator certified 6 months before. In the moment of the accident, the area where Mr Xu and the project manager were placed
was in the blind angle of the excavator mirrors.

WHAT HAPPENED: On May 26th, 2009, an employee of a third party service provider was fatally injured as a result of an electrocution during concrete
pumping activities. While pumping concrete for a storm water culvert on the access road to the quarry, the third section of the pump boom was apparently
positioned too close to the overhead power lines (7,900 volts). The electrical current bridged from the power line to the third section of the pump in an arc,
travelled to the ground via the fourth section and the placing hose and through the employee who was operating the nozzle. At the moment of the electrocution,
the victim was standing in wet concrete.

WHAT HAPPENED: Wadi 2 site. On 3rd July, 2010, an employee of a third party service provider was electrocuted when he contacted a terminal on an electric
welding machine (manual metal arc welder -MMA). The victim was working as a welders assistant at the time of the incident. At approximately 12:40 hrs, the welder and the
victim were leaving their work area for a break (lunch break). The welder has handed over his welding helmet and gloves to the victim so he could store these items during their
break. The victim has placed the welders equipment underneath a steel corrugated sheet which was lying next to the the welding machine. It appears whilst returning the victim
has stumbled and fallen, in which his right hand has contacted the welding machines secondary output terminal. The combination of contacting the live terminal and wet
ground conditions has allowed the current to pass through the victim to ground, resulting in him receiving a fatal electric shock.

WHAT HAPPENED: Shurovo site. On March 7, 2009, at approx. 09:45 a.m., two surveyors of a 3rd party service provider conducting a topographical
survey. The workers were caught by a landslide while effectuating measurements in a trench of the Shurovsky expansion project. The trench had a depth of 5 m,
a width of 9 m and a length of approx. 100 m. Their supervisor was standing on the top of sewage well . One of the surveyors, aged 52 was buried by the land
slide and died immediately. The other surveyor was not injured and could leave the hospital the same day. No other works were performed in the vicinity of the
accident.

WHAT HAPPENED: Tetouan L2. On January 17, Allal Sarout, aged 35, married with 2 children, contractor’s mechanic, was killed when crane block fell directly onto his
head. The accident occurred on the crushing shop construction site. At the start of his shift, the victim and five colleagues were preparing to insert a shoot in a conveyor duct.
They were standing at the bottom of the screening shop retaining wall. Positioned at the top of the wall was a crane belonging to a specialist lifting gear contractor working for the
construction company. The crane was set in motion by the operator of a cherry-picker, who was not qualified as a crane driver but got access to the keys of the crane .
As he was deploying the jib of the crane, he felt the crane move and though it had become unstable. Then he climbed down to check the stabilizers without completing the
extension of the jib. At this point, the cable snapped. The unladen block and hook fell directly onto the head of the victim, whose hard hat was unable to protect him. He was killed
instantly.

access to hazardous locations.

WHAT HAPPENED: Chanda site. On 30th June 2009 at approximately 17:45hrs, an employee of a third party service provider was fatally injured when he was
electrocuted during a lifting operation. The incident occurred when the deceased was assisting with the unloading of the casing to the kiln feed bucket elevator at the plant
storage yard. The casing was unloaded from a truck and transferred to its storage area by a non slewing mobile crane. As the crane approached the storage area, the raised
boom of the crane contacted an overhead 11kV power line. This contact with the power line caused the current to go to ground through the crane boom, its load and then
through the deceased who was guiding the load into position .

WHAT HAPPENED: Bonaberi plant. On April 17th, Moussa MOUNDI, aged 24, worker of a civil works subcontractor, died from electrocution. The task oh his
team was the demolition of an existing wall. At 11:00 site management members instructed to the team to continue demolition by avoiding an area where a visible
electrical cable was located. The supervisors left the site and then Moussa I took the jackhammer from his colleague who was tired. For an unknown reason, he
moved towards the cable and started to demolish the rubble around the wall. The jackhammer touched the 380V live cable and Moussa was electrocuted. His
coworker alerted the shift supervisor who proceeded to the electrical isolation of the plant area. After assuming the isolation, the victim was evacuated.

WHAT HAPPENED: Woodstock plant. On August 23rd, 10:00 AM, Walter Nuvoloni, aged 47, worked at Holcim Canada for 16 years in Procurement,
died after the fall from an elevated catwalk. Walter was walking through a section of the decommissioned cement plant with a group of contractors when
he fell from an elevated catwalk, sustained significant injuries. He was immediately transferred to a local hospital for treatment but he succumbed to his injuries
later that evening.
Further investigations revealed that a section of grating gave way when it was
walked on.

spot.

WHAT HAPPENED: Al Safwa L2. On July 6th, Mr.Huyat Ur Rehman, aged 33, a TUV Certified Pump Operator and his helper had just finished concrete
pouring for the new crusher project. They were working for a concrete supplier, sub-contracted by the project main contractor. They started demobilization while the
supervision team went to the shelter to rest (hot weather and Ramadan period). He started cleaning the pump back, while asking his helper to close the outriggers.
Mr Huyat was crushed between the outrigger and the pump body, as he was operating a cleaning control panel from the ground in between the outrigger and its
pitching point. He was rescued still alive by a normal pickup, but arrived to contractor clinic 40 minutes later with no vital sign.

● WHAT HAPPENED: Damodhar grinding.. On February 26 th , Sukumar, a 55 yo employee, maintenance mechanic of mobile equipment died at his working
place.
● Sukumar was cleaning the filters of a road sweeper machine. During this task, the machine operator, who was in his cabin with the machine in energized
condition, closed the front flap which was controlled through a hydraulic cylinder, trapping Sukumar between the hopper and the flap.
● Sukumar was immediately shifted to ACC-Occupational Health Centre and then shifted to a specialized hospital 35 km away from Damodar Plant. He passed
away on the way to the hospital .

● WHAT HAPPENED: Bouskura cement plant.. On January 21 st , Mr. Danoune Hassan, 43 years old contractor, was fatally injured by a belt conveyor.
● A team worked on the replacement of the belt conveyor under the primary crusher from 00 am to 9 am.
At 9 am, another team, including Hassan, took over to complete the work. During the test phase Hassam approached the belt conveyor, was caught by the belt
and died immediately. The reasons why he approached the belt conveyor are still under investigation.

● WHAT HAPPENED: St Appolinaire. On September 14 th a contractor worker was fatally injured while he was cleaning the inside of the drum of a ready-
mix concrete truck. The job consisted in removing the hardened concrete from inside the drum with a jack-hammer to prevent road accidents due to an over
weighted truck. For safety reasons and due to the toughness of this activity, the job is always performed by 2 workers. In this very case one of them was the
truck driver and the other one was the victim. The victim had entered the drum partially through the man hole, when the drum started to rotate for
currently unknown reasons. The victim had already performed this job several times since June 2010 and was recognized by his colleagues as professional.
Furthermore, a specific safety procedure is in place in Dijon Beton to deal with this particular job.

● WHAT HAPPENED: CALALBAR L2 project, Contractor mechanical team taking measurements in preparing to install cladding /rain cover for an
additives stacker belt conveyor without any PTW in place. The team leader only gave instruction to the workers (in the TBT) in how to activate the
emergency stop of the belt conveyor but not about LOTOTO procedure.
● One of the workers was standing on top of the belt with no lock out, Line 1 operation warning alarm sounded and also Line 1 personnel warned the
Chinese worker to get off the belt, but he refused to step down because he didn’t understand the warning and he didn’t register the alarm sound. The
belt started and drove Mr. SHI LU LAI for about 2 meters before the local stop pull cord was applied by one of the vulcanizing team members of Line1. The
belt stopped and there were no injuries.
KEY SAFETY MEASURES. The security of all suspended loads during lifting operations must form an integral part of the lift planning assessment.
Additional control measures such as cages, boxes or nets must be provided to ensure all potentially loose loads are secured. Wherever possible a minimum of
two securing points should be made on all loads.The person controlling crane movements and receiving the load must ensure safety of all persons in the
immediate vicinity of the load being lifted. Risk
assessments in relation to future lifting activities involving potential loose objects, such as slip-forming, should be reviewed to ensure the security of all loads.

WHAT HAPPENED Hagerstown Plant, Holcim (US) Inc., December 3rd 2014at 3:30am, 3rd Party employees were removing liner plates inside a ball
mill. Normal mill ventilation systems were down for maintenance and auxiliary fans were in use at the mill door openings. Gas monitors were being used in the
work area but were not directly on the person performing the cutting. Carbon monoxide gas accumulated from the carbon arc cutting inside the mill
causing the contract employees to become symptomatic (nausea and headache) by the end of the work shift . During the shift the gas monitor went into
alarm several times as reported later by the contract employees, but not logged by the hole watch person. During these alarm events the workers did not exit
the mill to determine the source of the carbon monoxide gas. Two of the exposed persons were treated at a local hospital and released later that day. The
air monitor was bump tested prior use and calibrated on a regular basis.
KEY SAFETY MEASURES. Ensure proper ventilation in areas where welding / cutting will take place. Carbon-arc cutting creates carbon monoxide.
Communicate all abnormal confined space events or alarms to supervision. Verify understanding of confined space roles and responsibilities including hole-
watch or other specific positions. Exit the work area during gas monitor alarm situations and determine the cause before proceeding. Ensure all workers properly
understand the serious issues from exposure to unsafe levels according to the gas monitor. Don’t simply ignore the alarms!
