Safety Meeting Minutes 2/10/2015

Action Items Open/Follow-up:

Safety Program Reboot the safety program with an emphasis on incentives for
active participation versus the passive card system. Reviewed Fremont Safety program
and will move forward with gift cards for completion of Otis safety training.

Plate Bins Tracy Kuhn updated Committee on progress. Adding different style
Bin, (Tracy Kuhn) New turntable type bin (On hold), Table installed (Lehsten).

Traffic Hazard ID Mapping One more mirror needs installed, Rework doorway Need
guardrail installed

Fire Alarm System Add another Amplifier and speakers for Fire alarm and make an
audible alert on paging system. Torrence Sound in Engineering and quote phase. (Tim
Hull) Nothing done on this. Awaiting on Torrence Sound
o Evacuation Rally Areas Place Placards for each area (FE, BE, QC,

Washer Training on Handling Chemicals Look into training (Minor/Stewart)

Kone Cranes Safety Summary List. Has hoist that is rated for 1 ton but Beam is
only rated at ton. Repairs - 6 new items from monthly audit In Process

Order New Safety banners emphasizing personal ownership and elimination of

Pal/Depal Anti-fatigue mat Jon Elsie getting dimensions so mats can be

Completed Items:
Washer HF incident and concerns reviewed. To assist emergency
personnel 2 additional tubes of Calcium Gluconate will be purchased (Stewart
2/21). Universal Precautions will be reviewed with the team as multiple EEs
work on the washer besides the trained washer mechanics. Suggested that
this become part of the annual safety refresher training (Safety Committee
2/28, curriculum/materials TBD) Inspect and verification of Chemical Storage
Room ventilation. Tubes in Washer Lab & Production office. (Ray
Stewart)

Hard Hats - Some people are wearing, some are not-all agreed policy has not
change and must be enforced consistently and uniformly across crews. More
Caps on the way
Sprayer Ladders (2) two step ladders purchased. No handrails on them.
Look into another type of ladder. New ladders presented at meeting. Ladders
in place and in use
Sprayer Brakes Electrical changes made to stop them quicker and Brakes
in process of installation. Adjust decal to cure drive faulting. Completed
Safety Jackpot Issues with no card being handed out. Continuing
Doors around plant being left open Enforce SQF Policy (All Supervisors
and employees to enforce)
Basecoater 1 Catwalk needs finished on mezzanine. (Tom Lehsten
completed)
Back End Job Box- DeWalt grinder removed from Job Box
Necker Tooling Copper Sulfate being used in Kent, Washington for cleaning
Tooling. Continue with our current process.
Site Security went over visitors knowing gate code.
Deco Guarding Head bump issue, soliciting ideas and suggestions from the
mechanics. Recommendations sent to RTC and in house adjustments made
when possible. No feedback reported on this yet. (Hull/ Owen)
Blades being used for Date codes continuous use of blades to remove
date codes for plates use retractable knives. Come up with a different type
of application. (Ray Stewart, Jeff Nyitrai, Tracy Kuhn) Enforce Knives
being used.
Lockout / Tagout Went over new procedure, removal permit, and device
labeling
Item brought for shift to shift hand offs. Continue process and investigate
procedure for handoff (Tim Hull & Jon Elsie)
Chemical Room Rack Storage Racks too small. Need to investigate
(Adam Wilson)
New Water Cups for Floor personnel Get cups (Alisha Craig & Tom
Lehsten)

Uncoiler Material Stand Come up with a guard for the Coil stock as it rolls

Deco jog sequence needs a permanent fix as the current configuration has
no indexing or inch control. Contact RTC about the issues and
suggested/recommended fixes. This is a corporate initiative that will need
approval. (Chestnut 3/14). Approval given for jog sequence changes. (Chesnut
& Engineering). Change was made and process is under review for safety and
functionality. Complete
