Band and Choir Polo Questionnaire and Order

As festivals and future concerts approach, we need
to order band and choir polos, which have been
traditionally been the uniform for 7th and 8th grade
ensembles.
Every student should fill out the form below and
return to their music teacher by Friday, February 6. If
you are purchasing a polo, please include a check for
$15 to Schoolcraft Community Schools.

I already own a black Schoolcraft Music Polo that
fits and is in good condition.

I do not have a black Schoolcraft Music polo and

will purchase one. I will include a check for $15
(to Schoolcraft Community Schools) with this
form and turn it in by February 6.
