We ask that you participate in the evaluation of the candidate in one of two ways. 1. You may record narrative comments which you feel would aid in the evaluation of the candidate's experience on this form with attachments. Please include any statements which adequately explain any evaluation protocol or format you choose to utilize. 2. You may write a letter to the candidate, congratulating him/her on the successful completion of the practicum experience and thanking the student for the contributions that were made to students, the school and/or the community. You may add any other information that you feel is important and relevant. Once you have selected your preferred method of evaluation, please return the completed document to the University Supervisor at the end of the practicum experience.
