The faculty at Frostburg State University's School for the Educational Professions thanks you
for serving as an On-Site Mentor to a Master’s Degree candidate in the field of Education
Leadership. We ask that you participate in the evaluation of the candidate in one of two
ways.
1. You may record narrative comments which you feel would aid in the evaluation of
the candidate's experience on this form with attachments. Please include any
statements which adequately explain any evaluation protocol or format you choose
to utilize.
2. You may write a letter to the candidate, congratulating him/her on the successful
completion of the practicum experience and thanking the student for the
contributions that were made to students, the school and/or the community. You may
add any other information that you feel is important and relevant.
Once you have selected your preferred method of evaluation, please return the completed
document to the University Supervisor at the end of the practicum experience. The
University Supervisor will submit your report to department officials for appropriate filing in
documentation of an internship.

‘Congratulations on your milestone achievement of completing your practicum experience! As an
Assistant Principal and your mentor, | realize the hard work and tremendous amount of time needed to
achieve a Master's Degree in Educational Leadership,
First and foremost, | want to thank you for your time spent with Allegany High School students. | have
witnessed compassionate, knowledgeable, and open-minded responses when they, at times, do not
always warrant. Your ability to reason and handle difficult people and situations will enable you to be a
successful school administrator.
Secondly, your interactions with teachers and staff were always professional, helpful, and caring while
also listening to the needs of teachers, You are an instructional leader who works collaboratively with
colleagues to improve instruction with the ultimate goal of enabling students to be college or career
ready.
Jessica, | am extremely grateful that | worked with you during your practicum experience as you
provided me with excellent insight into educational leadership qualities when dealing with students,
parents, staff, and the community of Allegany High School.
