To inject the N2 into the fabricated ball valve and every connection to conduct leak
test, after completion of the works purging out the N2 from the valve.

Mitigation Measures (Construction):
1. Lifting crew must competent and appointed
2. To conducted pre-start briefing to lifting crew by lifting supervisor
3. Signal man and excavator operator communication language is both understand
4. LM/LG good condition and valid certificate available on site for verification
5. To determine the CG before proceeding to lift up the load
6. rigger to move away from the load after rigging completed
7. PTW & Lifting plan available on site and lifting works must be according to lifting plan.

1. Safety gloves to be worn

4. Do not obstruct the access/ work area by materials.
5. Ensure all test equipment comes with calibrated certificate prior to install
6. QC supervisor to be double check every fitting after installation and connection
7. To comply pre-checklist for valve passing test.
8. Competent personnel to do the task
9. QC to do a line check at every connection before allowing to start the pressurization.

1. To notify relevant parties prior to start the works
2. PE to inspect and endorse the valve testing process.
3. Assigned competent worker to do the job
4. Ensure the manifold , connectors used in good condition
5. To install whip arrestors at every connection
6. QC supervisor/ Inspector to do final check after every fitting to check for loose connection
7. To use soap solution to check the leak
8. No unauthorized personnel in the testing area

Mitigation Measures (Construction):


1. During depressurization, the N2 must be release upwards ( vertically) to the sky to the air.
2. The valve must be fitted pointing upwards ( vertically) to the sky.
3. Worker to wear safety goggles , ear plug and hand glove
4. QC inspector to be on site when the work in progress
5. To notify relevant parties prior to do the works.
6. Control the venting to reduce the noise level
7. To wear ear plug
8. Competent person with mechanical background to do the job.

Emergency Response

During an emergency all site staff are to evacuate to site Assemble Area (CAA) outside
of the site at side table.(Displayed emergency assembly point signage)
1. Immediately stopped the work activities
2. Evacuation are to be in orderly manner
3. Site supervisor for each location has to evacuate every workers out from the site
4. Perform a headcount or check to ensure that all workers are safely accounted.
5. Project Manager are to reported to Ley choon management and SPPG & WP

Reporting
Project Manager shall proceed to emergency site within the shortest possible time
to carry out his preliminary investigation.
He shall report to WP/SPPG any other governmental agencies.
The incident must be reported to the applicable agencies for incident involving
injuries to workers and loss insurance agency. Incident not involving injuries shall
also be reported WP/SPPG.
