    1. A near miss incident occurred at the TG deck erection site of IVRCL's NTPC Solapur project. While lowering a 9 meter, 300kg crib column with a 50 ton crane, it struck an unsecured crib column, causing 4 crib columns to collapse. 2. The immediate cause was that 5 crib columns were not properly supported at the condenser site. The root causes included a lack of supervision, failure to follow PTW procedures, lack of competent riggers, unsecured crib installations, and no prepared method statements or HIRAs. 3. Immediate actions taken were to issue a notification to the contractor and provide support for the crib installations

    Brief Description: Around at 9:15 am at TG deck erection site. While an erection of cribs coloumn
    (length 9 meter, approximately 300 kg weight) was being lowered to the condenser ground for the
    purpose of installation with the help of ( capacity – 50 ton) tyre mounted crane. Before incident they
    had already installed 200 numbers of cribs coloumns at site. All the cribs coloumns were in the vertical
    position with properly supported. but 5 numbers of cribs coloums were not supported properly at
    condenser site. While erection times the bottom part of the cribs colomn was striked to another non-
    supported crib coloumn. Suddenly 4 cribs colomns were collapsed due to strike of erection crib
    coloumn. That’s why the incident was occurred. No personal injury is reported that incident time.

    Immediate cause: Inform concern engineer and concerned agency to provide/support properly.

    Root cause: Lack of supervision.


    PTW system was not followed at site.
    Competent riggers are not available.
    Installations of cribs are not supported to another crib.
    Method statement and HIRA was not prepared.
    Basic rigging plan must be in place
    Guide rope was not properly tied with the cribs coloumn.

    Remedial measure: Immediate provide support to installation of cribs

    Immediate Action Taken: Notification has been issued to M/S New Sai.

    Recommendation: Installation of cribs must be supported to other cribs.

    Lesson Learnt: First make proper arrangement then start the activities.
    Always erect 2-3 cribs during installation time.
    The installation of cribs should be supported to another crib.
