Ako, si NADINE REID, labing apat (14) taong gulang,

estudyante, naninirahan sa Graceville, CSJDM, Bulacan, sa harap at
kaalaman ng aking ama na si Enrique Reid, makatapos na
makapanumpa ng sang-ayon sa batas ay nagpapahayag, ng mga
sumusunod:

Ang nagtatanong sa akin sa wikang Tagalog ay si Atty. Abo

Gada na may pahatirang sulat sa Room 202 Roxas Bldg., Mojon St.,
Malolos City. Ang pagtatanong/eksaminasyon ay kanyang ginawa sa
nasabing opisina at sinagot ko ang kanyang mga tanong ng buong
katotohanan lamang:

 QUESTIONS AND ANSWERS

1. T: Kaugnay sa kasong ito, may naaalala ka bang ginawa na

sinumpaang salaysay?
S: Meron po.

2. T: May ipinakikita ako sa iyo ngayong sinumpaang salaysay

na may markang Exhibit "A”, ito ba ang iyong sinasabi?
S: Ito nga po iyon at ako nga po mismo ang nakapirma sa
Exhibit "A".

3. T: Noong ika-21 ng Mayo taong 2017, ano ang pangyayari

na iyong naaalala kung mayroon man na nagresulta sa
reklamong ito?
S: Tinamaan po ako ng bato sa noo dahil sa pagbato ng
kapitbahay namin na pangalan ay James Lustre.

4. T: Nasaan ka nung naganap ito?

S: Natutulog po ako sa kwarto nila mama at papa.
Nakahiga malapit po sa may bintana.

5. T: Ano ang nangyari sa noo mo?

S: Naramdaman ko na lang po na may tumama sa noo ko
kasabay po ng malakas na tunog. Naiyak po ako sa sakit.

6. T: Ano ang sumunod na pangyayari?

S: Nagising po ako at minura pa po ako ni James Lustre na
sumisigaw sa harap ng bahay namin.

having been duly sworn to in accordance with law hereby depose and
state that:

1. I personally conducted the examination of Nadine Reid

under the supervision of her father Enrique Reid;
2. I faithfully recorded the questions I asked her and the
corresponding answers that she gave; and
3. Neither I, nor anyone, coached her regarding her answers.
