Good day Sir,
This is in connection to numerous incident that happened along the National Highway of Sitio Atipolo,
Pob. Zone II, Capoocan, Leyte particularly at the uneven surface of pavement/asphalt that causes the
motorist/motorcycle rider to slide. Obviously, part of your program is to safeguard all motorist in order
for them reach to their destination safely.

In this regard, I would like to request your good office to conduct survey/inspection and take an
appropriate action to the aforementioned place to prevent similar accident within the area of
responsibility.
