Dear Student-Trainee:

You are now ready to go to your respective training station. While you are in your
training station, please observe these guidelines. They are intended for your protection and
worthwhile stay in the company/ agency.

1. Follow all rules and regulations of the company/ agency regarding the training
program.

2. Maintain the good image of the University while in the company/ agency through
your work performance and courteous attitude towards the personnel.

3. Promote a good relationship between the University and training station.

4. Stick to your work assignment and never leave the company/ agency without the
knowledge and sanction of the University.

5. Communicate with your Faculty-in-Charge and/ or the Director of the IRJP on

problems encountered while on training for providing relevant and factual
information.

6. Accomplish IRJP Form regarding industrial activities on a day to day basis and
submit such reports at the end of the month to your Faculty-in-Charge.

7. Secure from the company/ agency your Certificate of Completion of training at the
end of training program.

8. Immediately one week after the training, submit to your Faculty-in-Charge the
following requirements.

a. IRJP form for Daily Work Activities

b. Evaluation Sheet
c. Certificate of Completion
d. Narrative Report

9. Attend the post training as schedule after your training period.

10. Good Luck and Happy Training.

 Failure in my part to submit in due time these requirements will mean no credit
and will be required to repeat the program.

dropped and will repeat SIT in the succeeding semester.
