Case : ms mayang ( 21 y.o ) has been hospitalized since 2 days ago with the burn injury at her
back. The doctor said to change her position.

1. Orientation Phase
“ good morning miss “
“ let me introduce my self, my name is Mayang Sri Wulan, You can call me nurse
mayang, i am nurse on duty today from 8 a.m to 3 p.m “
“ what’s your name miss ? and your birth date ?”
“ how are you feeling today miss ?”

Contract
“ i am going to change your positon now, about is 15 minute in order for your body

2. Working Phase
“ oke miss let’s start it, first i will to take a pillow”
“ miss, i am going to place the pillow the top your bed, i am going to put your hand on
your chest and bend you right knee and i am going to reserve your position br prone
position”
“ i need to decline your head so that you can breath well.”
“i need put the pillow under your head but not to the shoulder, i need to put the pillow
to under stomach starting from the diaphragm ro the chest and place a pillow under
the feet, starting from the heel to the knee.”

3. Termibation Phase
“ how do you feel after changed position miss?”

“ i need going to report your candition and i will back at 1 p.m to give you medicine”
