SWORN AFFIDAVIT

I, THELMA VIVAR, of legal age, married with residence at 1045 Pitong

Gatang, San Dionisio, Paranaque City after having been duly sworn to in
accordance with law, do hereby depose and state:

1. That I personally known Maria Jinky Puche since 2011.

2. That we were also became co-workers at LCRM Manpower Services.

3. That due to our friendship, I frequently visited her at their home in

Caloocan.

4. That during my visits, I happen to know her husband SUNDAY PUCHE,

who is working as a civil engineer.

5. That sometime on 2012, Jinky told me that her husband will be

working in Ilocos Norte for 2 months for a certain project and she will be
transferring her residence at Quezon City. Jinky also told me that her
husband took back his atm from her possession prior to his departure.

6. That after 2 months since Jinky told me about the work of her husband
in Ilocos Norte, she told me that her husband did not return home. She even
told me that her husband did not give nor send her any money for her
support particularly the payment for their rent. Jinky further told me that her
husband never contacted her nor informed her about his whereabouts.

7. That I personally saw Jinky’s hardship in life since her husband leave
her in 2012.

8. That up to present, when me and jinky had a chance to visit each other,
I learned that her husband(SUNDAY) never contacted her nor give any
money or anything to support her.

9. That I am executing this sworn affidavit to establish the real status of

Jinky Puche since her husband left her without giving her any support or
anything that is due her whatsoever.
