RE : NOTICE TO EXPLAIN: INSUBORDINATION

Incident Report:
During my round at the Grade 9 Level on June 10, 2019 at 10:30 am, Grade 9
Aquamarine students are scattered outside of the room and rowdy in their room since
you were not present. The incident also caught the attention of the adviser, and asks
the student to look for you to attend the class. As to my action, I contacted you over
the phone to ask of your whereabouts on that moment; you answered the phone at
10:42 am, and gladly tell that you are outside the campus. I didn’t ask for the reason
of not coming to your class but request you to attend to your class immediately.

Around 5:10 in the afternoon, I humbly ask you for the reason of not attending the
class on time. You explained to me that you are having a diarrhea and not well. I told
you if that is the case, that next time is to give classroom activity or seatwork so that
students will not disturb other classes and we agreed that you will sign the PMCF
regarding the incident the next day.

Today, when we met to discuss and for you to affix you signature on the PMCF, you
were defensive by outpouring your emotion and told me that I’m not being inhumane
for such action hence you refuse to sign the PMCF. You were asked to sit and calm
down but you refuse. You are questioning the purpose of the PMCF. I pointed out and
clearly explain the purpose of it and still you insisted not to affix your signature. As
the conversation progress, you brought up some personal issues about the
management, your relationship to you colleagues and to me as your immediate head
which is merely based on emotion.

CSC Rules and Regulations:

The behavior you exhibited is a violation of Civil Service Rules and Regulations on
Administrative Discipline: Insubordination for the refusal of affixing your signature on
the PMCF which clearly states its purpose.

Action to be taken:
Submit your written explanation within forty eight (48) hours upon received of this
memorandum. This will give you an opportunity to explain to defend yourself and
provides you due process. Failure to submit within the deadline means you have
waived your right to be heard and management may make a decision.

Please be guided accordingly.
