Dear Sir,

I am writing to you this letter to explain on why I was not able to process my INCs and NFEs on
the given deadline which is August 22, 2019. As you know, I was diagnosed with Major Depressive
Disorder last June of this year. I was prescribed with medications and therapy for months. This
has greatly affected my studies and my daily routine as a student where I was even struggling to
just finish the last term to the point that the OSA office have also questioned my enrolment as to
I have occurred many absences to which I did not like to happen in the first place. The Student
Counseling Office is also aware of this as I also attended counseling with them and hoping for the
fast recovery.

Due to this illness, this 1st term enrolment, I have made a tough decision and came up to take a
break from school as I feel like the pressure is too much for me to handle for the upcoming term.
I might be just dropping subjects and might be failing to attend classes normally. My family
decided for me to take a vacation break at our provincial house where there unexpectedly my
grandfather accidentally slipped in our house that brought him to hospitalization. After a week,
it has brought our family a devastating news as to our grandfather passed away dated August 10,
2019. We, grandchildren were very close to our grandparents since they were the one who took
care of us since we were still young so this untimely demise of our lolo deeply saddens us.

As to the completion of INCs and NFEs, as a transferee student and first time to process this form
I was not familiar with how things usually be done but I have read on the back of the completion
form that according to the student handbook there will be a one month allotment from the start
of classes for the next term for the completions to be processed. I have relied to this statement
which gave me the thought that I still have enough time to process completions after the burial
of our grandfather which was August 22, 2019.
But as of August 20, 2019 my former classmates sent me a photo message which says that the
final deadline for completions will be on August 22, 2019 and any completions after that will not
be entertained. So I traveled immediately to Baguio to clarify things and there I was given until
August 24, 2019 to submit all my requirements.

Together with this letter, I have also attached supporting documents.

In this regard, I hope that you will understand and accept my completion.
