to be my true and lawful attorney-in-fact, for me and in my stead, to do and perform the following
acts and deeds, to wit:

1. To request, receive and/or claim in my behalf the ATM Card of my recently opened account
from the Bank of the Philippine Islands (BPI), Cagayan de Oro City; and

2. To sign any papers relative thereto and to perform all necessary and incidental acts in order to
carry out the above mentioned powers thereof.
