1.0 PURPOSE

To establish work guidelines in relation to SHE and security policies and


procedures for contractors and their workers performing jobs within the premises
of EMERY OLEOCHEMICALS TPG.

2.0 SCOPE

This procedure shall apply to all Contractors / Sub contractors and their workers
performing jobs and contractors in Emery Oleochemicals (Telok Panglima
Garang). The areas consist of:

representing a company appointed by Emery Oleochemicals TPG to
perform a certain job. Generally, the contractor will appoint his own to
perform such jobs.

Control.

5.0 JSA – Job Safety Analysis.

5.0 RESPONSIBILITY

5.1 SHES – SHE Manager and department is responsible to ensure this


procedure remains current and updated. Conduct SHE briefing to contractor.
Conduct audits of in process contractor work to ensure compliance to this
SHES procedure. Review submitted JSA or HIRADC by the contractor.

5.2 Plant & Maintenance Management – is responsible to ensure that all


personnel related to the work shall comply with the requirement of this SOP.

5.3 Project Coordinator - Define areas and job requirement to contractors,


Implement and ensure compliance with SHES rules during work.

5.4 Purchasing - Ensures that all contracts include language to require


contractor to comply with SHES policies and procedures.

5.5 Contractor - To uphold and comply with all SHE rules and regulations at all
times. Ensure all personnel have attended required contractor SHE briefing
prior commencing work. Sign and ensure compliance with Agreement on
SHE Code for contractor EOM.SOP.SH06 - F01- 01.

5.3 All personnel and contractors are responsible for following all requirements
outlined in this procedure and any related procedures.

5.4 All personnel are responsible for ensuring strict enforcement of all
requirements within this procedure and if necessary do audit the worksite for
compliance.

6.0 PROCEDURE

6.1 Security Rules

6.1.1 All necessary particulars i.e. full name, MYKAD number and areas of
work shall be recorded by the Security prior to issuing of contractor
pass.

6.1.2 If the Contractor is bringing tools, he/she is to declare them at the


guardhouse. This also applies to any other personal effects that
may bear resemblance to EMERY OLEOCHEMICALS TPG property.
The tools and equipment are to be approved by maintenance
department.

6.1.3 Prior to commencement of work, contractors/project coordinators


must submit workers name list to security.

6.1.4 If the contractors workers are foreigners, document required are:-

i. To produced original passport at Security Office for verification.


ii. Copy of valid working permit to be presented at the Security
Office

6.1.5 The contractor where possible shall be restricted to the allocated


work area and shall not loiter around the premises.

6.1.6 Should the Contractor need to remove EMERY OLEOCHEMICALS


TPG property, a gate pass shall be obtained.

6.1.7 The Requestor / Project Leader shall be responsible for obtaining


clearance for the job within the relevant plant engineering
department.

6.1.8 No vehicle is allowed into EMERY OLEOCHEMICALS TPG premise


except with permission from security. Logging In – all visitors to
EMERY shall come to the Main Security Post and sign the visitors
log immediately upon arrival.

6.1.9 Hazard/Emergency Plan Notification – as part of the log-in process,


visitors, suppliers, vendors, drivers and contractors will be given
safety and health briefing (refer to EOM.SOP.SH03 Briefing of SHE)
describing the hazards of the workplace & the procedures to follow
in case of an emergency.

6.1.10 Personal Protective Equipment – all visitors, suppliers, vendors,


drivers and contractors must use & wear PPE at all times while
visiting the plant or at the PPE zone.

6.2 SHE Briefing / Induction

6.2.1 All contractors’ employees and sub-contractors are required to


attend Health and Safety induction conducted by EMERY
OLEOCHEMICALS TPG on every Tuesday and Thursday prior to
commencing any work. The name list of participants should be
submitted to security Guard house ‘A’ one day before the day of
induction to be conducted.

6.2.2 Once SHE briefing completed, they will receive Workers Briefing
Pass that will valid for 1 year and they need to attend a refresher
SHE briefing once the card expired. Please refer to EOM.SOP.SH03
Briefing of Safety and Health procedure.

6.3 Reporting of Incidents

6.3.1 Contractors must immediately report to Emery Oleochemicals TPG


of any incident, injury, fatality, occupational poisoning, occupational
disease or dangerous occurrences, property damage and
environment pollution at the premises using the reporting
procedures set by Emery Oleochemicals TPG. Contractors must
also assist Emery Oleochemicals TPG in the investigations and
undertake improvement measures to prevent future occurrences.

6.4 Working Hours

6.4.1 Working hours for contractors is from 0800hrs to 1900hrs, if they


require extending the work after 1900hrs they need to fill in the
overtime form that is provided by security and get approval from
Emery Supervisor / Superintendent as required in the form. However
the work can only be extended until 2300hrs provided to comply with
permit to work procedure. Special approval is required from related
Head of Department for work to be extended beyond 2300hrs.

6.4.2 Lifting work using crane and working at height are not allowed after
1900hrs.

6.4.3 For weekend work or work during public holiday, the contractor
needs to get an approval by requesting weekend work application
form from project coordinator by fill in EOM.SOP.SH06-F02- 01.

6.5 Certification by Regulatory Authority for Occupational Safety and Health

6.5.1 Before contractors are permitted to work at the company’s worksites,


they must present proof of good standing with the relevant
regulatory authority on Occupational Safety and Health particularly
for those workers requiring certificates of competency, and must
maintain their good standing for the duration of the contract.

6.6 Safety Plan / Method of Statement (only for non-routine work or major work)

6.6.1 Prior to the commencement of work on site, the sub-contractor shall


present to the company a detailed method statement incorporating a
plan for safe systems of work and explaining how safety will be
managed and controlled throughout the contract works. This plan
shall comply in every respect to the Company Health and Safety
Regulations. It should specifically detail how the Contractor intends
to implement.

6.7 Hazards Identification, Risks Assessments and Determination of Control


(HIRADC) or Job Safety Analysis (JSA).

6.6.1 All work activities done by contractors and sub-contractors should


conduct risks assessment and approved by area owner or project
manager before they proceed with the work. They can use risks
assessment method provided by Department of Safety and Health
(DOSH), Emery Oleochemicals (M) Sdn. Bhd or any format that they
are adopting and practicing at their workplaces. Every risks
assessment should be submitted to Emery Safety and Health
department 7 days before commencement of work.

6.8 Declaration of Equipment / Tools

6.8.1 All work activities done by contractors and sub-contractors should


conduct risks assessment and approved by area owner or project
manager before they proceed with the work. They can use risks
assessment method provided by Department of Safety and Health
(DOSH), Emery Oleochemicals (M) Sdn. Bhd or any format that they
are adopting and practicing at their workplaces. Every risks
assessment should be submitted to Emery Safety and Health
department 7 days before commencement of work.

6.8.2 Supervisor in charge detailing the items and duly approved by HOD,
Production Manager or Maintenance Manager. The guard shall
check these items against the gate pass item prior to allow the
contractor to take out the items.

6.8.3 For item required to be sent out during public holiday / weekend, the
Gate Pass has to be prepared in advance with duly approved by
respective HOD, Production Manager or Maintenance Manager.
However, in case of emergency situation, with obtaining the
authorization from by HOD / Production Manager, Superintendent on
duty and Senior Guard on duty shall be responsible to sign on the
“Gate Pass”.

6.9 SHES Rules

6.9.1 Contractors shall observe all federal, state, local and facility SHES
regulations and procedures while performing work on the premises of
any EMERY OLEOCHEMICALS TPG facility.

6.9.2 The contractor should report directly to the project coordinator on all
incidents at site. Project coordinator must coordinate the incident
response with SHE department.

6.9.3 Every Contractor has the responsibility and obligation to know and
follow all facility rules, applicable laws and regulations and to make
health and safety an integral part of his daily work habits.

6.9.4 The Contractor is responsible for protecting the health and safety of
his/her workers or his/her subcontractors.

6.9.5 Every Contractor shall have a competent and responsible


supervisor/leader in charge at the site at all times during their workers
is on site.

6.9.6 Technical procedures for performing the work shall be provided to


workers by the Contractor. When applicable, procedures must be
available and communicated to worker on various methods such as :
6.9.6.1 Handling gas cylinders

6.10 Permits to Work

6.10.1 Appropriate permits to work must be applied for and duly


authorized by EMERY OLEOCHEMICALS TPG’s appointed
personnel.

6.10.2 Contractors must ensure that all conditions set out in the permits
are clearly understood and strictly adhered to.

6.10.3 Permit to work available in Emery is Cold Work Permit (refer to


EOM.SOP.SH30), Hot Work Permit (refer to EOM.SOP.SH15)
and Confine Space Work Permit.

6.11 Electrical Safety

6.11.1 All Electrical equipment’s must be inspected by Electrical


department and approved with Emery approval sticker before
used in the plant

6.11.2 No temporary joint wiring

6.11.3 All corded equipment must be inspected by the Electrical &


Instruments Department before use for electrical safety purpose.

6.11.4 Extension cords shall not be placed on busy walkway or wet floor
or on hot surface or equipment’s.

6.11.5 Switch off all equipment upon completion of job and before the
end of the work day.

6.11.6 Power connection must have proper plugs complete with ELCB
or RCCB (fitted with 5 Amps) and do not bare ends of cables.

6.11.7 All overhead electric lines must be noted by the Contractor when
working near such lines.

6.11.8 Extension cords should be protected especially hot work is being


carried out at the same time.

6.11.9 When working in defined ATEX area, explosion proof equipment


shall be use prior to work.

6.11.10 Cable used for single phase supply (240 volts) must be 3 core
cable and industrial type and for three phase supply (415 volts)
there must be a plug top.

6.11.11 Wire to wire connection for extension must be done with


connectors and tape.

6.11.12 Electrical equipment used for outdoor must be weather proof and
for hazardous areas must be explosion-proof type.

6.11.13 Cable layout across the road must be protected by G.I. Pipe.

6.11.14 If lock out/tag out is required for the equipment’s, please contact
Maintenance Department.

6.11.15 Contractor is not authorized to operate plant electrical system.

6.12 Hot Work

All welding, cutting, grinding, drilling and torch used by the Contractor or
any activities that produce spark must have an approved hot work permit
before work begins. The Contractor is to observe that:

6.12.1 All hot work permits are valid for one shift only.

6.12.2 New permits are issued on every new shift.

6.12.3 The hot work permit must be displayed on site at all times and
return to Project Coordinator before end of the shift or after the
completion of work.

6.12.4 Sparks must be prevented from falling on equipment, bare wires,


explosive chemicals, oil, petrol, paint and any flammable
materials.

6.12.5 All gas cylinders must be secured

6.12.6 Gas cylinders are to be kept away from any source of heat or any
electrical circuits.

6.12.7 Contractor is required to have fire watcher during hot work


activities and fire resistant tarpaulin/ fire blanket to prevent
sparks.

6.12.8 All welding equipment must be properly grounded to earth.

6.12.9 Fire extinguishers or water hoses must be available at the


working site at all times.

6.12.10 Person performing hot work is required to wear PPE such as


welding shield, leather glove.

6.13 Oxygen / Acetylene / Fuel Gases

6.13.1 Liquid Petroleum Gas (LPG) cylinders must not be stored or left
in areas below ground level overnight. The quantity of Oxygen,
Acetylene and Liquid Petroleum cylinders at the point of work
shall be restricted to a maximum of 1 day’s supply.

6.13.2 Compressed gas cylinders being stored, transported or used


shall be in an upright position and secured to some substantial
object or structure by a chain or other suitable means capable of
supporting the weight of the cylinder empty or full or be kept in a
cylinder rack or cage.

6.13.3 Cylinder valves shall be closed and the hoses depressurized


when the cylinders are not in use. Cylinders shall have protective
valve caps in place while they are being transported and
whenever they are not connected for use.

6.13.4 All cylinders shall be returned promptly to a suitable storage area


after use. They shall not be left around the worksites. Cylinder
storage areas should be away from general traffic paths and
must NOT be adjacent to vehicle paths. Flammable gases and

oxidizers must not be stored in close proximity unless separated


by a non-combustible wall. All hot work permits are valid for one
shift only.

6.14 Use of factory services (electricity, compressed air, water).

6.14.1 Under no circumstances no connection shall be made to any


service without a consent or approval from E&I and production
department.

6.15 Hazardous Materials

The following SHE measures are to be observed by the Contractor in


handling hazardous materials:

6.15.1 Contractors must provide a list of chemicals that they may use in
the facility and their corresponding Safety Data Sheet to the
facility management before commencement of work.

6.15.2 No chemical waste can be discharged in the ordinary trash


dumpsters, spilled onto the open ground, poured into any drains
of sewers or any other means that may destroy.

6.15.3 Any substance release should be reported to project coordinator.

6.15.4 Chemical container must be in good condition and labeled as per


local regulatory requirements after completion of the jobs, all
chemicals must be removed from the plant.

6.16 Personal Protective Equipment (PPE)

PPE shall be furnished by the Contractors and used properly by the


workers as required in the HIRADC or JSA submitted by the contractor.

All work tools/ equipment including PPE is to be supplied by the Contractor

6.16 Scaffoldings (tubular only)

6.16.1 Only tubular scaffolding is allowed to be used in Emery.

6.16.2 All Scaffoldings erected on site which is more than 3 meters


requires approval by a competent person (DOSH Certified).

6.16.3 Please refer procedure EOM.SOP.SH34 Scaffolding for the


details.

6.17 Trenching and Excavation

6.16.1 All trenching and excavation work must be carried out and
supervised by competent person(s). Particular attention must be

6.16.2 Contractors must obtain all available information about the


location of underground services before commencement of work
by complying with Emery excavation procedure EOM.SOP.SH45.

6.18 Confine Space Entry

6.18.1 Contractors should be trained before performing any confined


space task. They should have valid AESP (Authorized Entrant
Standby Person) and Health certificate issued by OHD
(Occupational Health Doctor)

6.19 Managing Emergency situations

6.19.1 If you hear the fire alarm ringing; you must prepare to evacuate
the building or area.

i. Switch off all computers, printers and electrical appliances.


ii. Gather your personal belongings in preparation to
immediately evacuate the building.
iii. Organize/help other people in the area.
iv. Evacuate the building and proceed to the assembly area.
v. Move at a quick walk, do not run

6.19.2 Some of the work or project will required the contractor to have a
suitable written plan to handle emergency situations that may
arise on the site. This requirement will be decided by project
coordinator during HIRADC or JSA discussion. This plan shall be
posted in the work place and the following is guidelines on
information that can have in the written plan:

i. Safe shutdown of all work activities.


ii. Detailed instructions for notification of the proper authorities
(including phone numbers, etc.)
iii. List of individuals responsible in organizing and controlling the
emergency conditions.
iv. Communication plan to ensure all the site personnel are
aware of their correct response to an emergency.

6.20 Non-Compliances and Violations Penalties System.

The applicable penalty for each offence is as set out below: …..

6.20.1 In modern OSH Management system, penalty system is the last


alternative approach to be considered. Adequate provision of
training and supervision are the most appropriately primary
approaches. Integrity of all level of personnel involved in this
project in ensuring the rules and requirement are adhered by
everybody is essential.

6.20.1 The primary objectives for the development of this guideline are:-

1) To warn and remind all parties about the seriousness of Emery


Oleochemicals (M) Sdn Bhd management in managing and
handling SHE issues
2) To increase the degree of enforcement of SHE rules and
regulation.
3) To evaluate the degree of SHE compliance

6.20.2 Payment of the penalty or penalties will be made by way of a


credit note issued by the Contractor and which the Client can use
to set off against the amount payable in any subsequent invoices
to be issued by the Contractor after being notified of the penalty
by the Client’s SHE department. In cases where no further
invoices are due to be issued by the Contractor after being
notified of the offence by the Client’s SHE department, then the
Client will issue an invoice to the Contractor for the applicable
penalty where payment for the invoice shall be made by the
Contractor within thirty (30) days of the date of the invoice .

6.20.3 This non compliances and violations penalties system is


applicable to all organization, companies work group/team,
vendors, suppliers and individual that involved in the project.

6.20.4 Any noncompliance will be recorded in penalty systems form and


the form (EOM.SOP.SH06-F03-01) will be submitted to finance
department for payment deduction from the project costs.

6.20.5 Types of SHE non compliances can be refer in appendix 3.

6.21 Terms and Conditions for Provision of Services

General compliance and safety, health and environment:

The Contractor shall comply with all applicable laws and provide
the Company with any permits, licenses, approvals, notices or
similar documents as requested by the Company for the
purposes of performing the Services. The Contractor shall itself
and ensure that its employees or agents comply with the
Company’s instructions and directions as advised by the
Company including those pertaining to safety, health and
environment as set out in the Company’s SHE Code of Conduct.
Where the Contractor contravenes the SHE Code of Conduct,
the Company shall issue the Contractor an invoice for payment of
the applicable penalty for the relevant offence which shall be
payable within thirty (30) days of the invoice date. The
Contractor shall bear responsibility for its safety and shall inspect
and properly use all safety equipment, safety clothing, tools and
construction equipment furnished by the Company where any
injury or damage resulting from such improper use shall be at the
sole risk of the Contractor. The Contractor shall be liable to the
Company for any damage, destruction or loss of any of the
aforesaid equipment furnished by the Company. The Contractor
shall take full and unequivocal responsibility for the adequacy,
stability and safety of the site where the Services will be
performed including any injuries or damage arising from
unsuitability or inadequacy of the site.
