1. PURPOSE

This procedure provides the minimum requirements for guidance to


inspector to perform Inspection of Pedestal Crane.

2. SCOPE

2.1. This Procedure shall be utilized for Certification & Re-certification of


Pedestal Crane.

2.2. Certification is conducted for fulfill the requirement of government


regulation.

2.3. Re-certification is renewal the government (MIGAS/DEPNAKER)


certificate.

4. PERSONNEL QUALIFICATION

Inspection for Certification & Re-certification of Pedestal Crane shall be


performed by qualified & certified personnel in accordance with SOP for
Personnel Training & Qualification.

6 PROCEDURE

6.1. DOCUMENTATION

Reviews of existing documents are necessary to have good inspection


result. The source of documents may be from manufacturers report ;
log book or maintenance record.
The Following documents required:
1. As Built Drawing.
2. Wire Rope Certificate.
3. Operation Manual Book.
4. History Card / Maintenance Record.
5. Load Chart of Crane.
6. Material Certificate.
7. Manufacturer Data Report.
8. Migas Certificate ( if any ).
9. Report of Repair, Modification and / or Replacement (if any).

7. STRUCTURE

7.2.1 Lattice Boom :


1. Dimensional check and compare to drawing and / or
document.
2. Cheek pin of pivot boom.
3. Check the condition of boom for sign of crack; bend and
corroded members.
4. Check boom angle indicators over full range for
accuracy.

7.2.2 Boom Tip :


1. Dimensional Check and compare to drawing and/or
document.
2. Check for the sign of crack, bend and corrosion.
3. Check for loose or missing bolts, pins connection and rivet.
8 .3. DRUM:

8.3.1 All drum (s) shall provide a first layer rope pitch diameter of not
less than 18 times the nominal rope diameter used.

8.3.2 The flange shall extend a minimum of' 1/2" (13 mm) over the top
layer of rope at all times.

8.3.3 Drum (s) shall have sufficient rope capacity with recommended
rope size (s) to operate within the range of boom lengths,
operating radii and vertical lifts as agreed to between the
manufacturer and the user/owner.

8.3.4 No less than two (2) full wraps of rope shall remain on the
drum (s) in any operating condition.

8.3.5 Check for cracked or worn.

8.3.6 The drum end of the rope shall be anchored to the drum by an
arrangement specified by the crane or rope manufacturer.

9 . BRAKES AND CLUTCHES:

9.4.1 Check control mechanism of brakes and clutches for proper


operations.

9.4.2 Check for leakage and damage in air and hydraulic system.

9.3.5 Verify condition of all braking systems.

9.3.6 The brake should have adequate capacity to smoothly stop 110%
of rated load from the maximum design speed in the
lowering mode while lowering.

5.5. PRIME OVER

5.5.1. Check prime mover for proper performance and compliance with
safety requirements.

5.5.2. Check condition of radiator

5.5.3. Check belts and chains for proper adjustment.

5.5.4. Check all fluid levels.

5.5.5. Check condition of fuel tanks :

1. Fuel tanks shall be equipped with filler necks and caps


designed to prevent fuel contamination from external
sources.
2. Drain shall be provided on fuel tank.

5.5.6. Check exhaust system :

1. Exhaust shall be equipped with a spark arresting device.


2. Exhaust gasses shall be piped to the outside of the engine
enclosure and discharged in a direction away from the
operator.

5.5.7. Lubrication :

All moving parts of the prime mover for which lubrication is


specified shall be regularly lubricated. Particular care should be
taken to follow manufacturer’s recommendations as to points
and frequency of lubrication.

5.6. WIRE ROPE

5.6.1. Visually all wire rope for deterioration and damage :

1. Kingking, bird caging, is crushing.


2. Check corroded or broken at the end connections.
3. Check that it is correct size and rope is properly fitted.
4. U-bolt clips shall be installed with U-bolt section on the
dead end or short end.

5.6.2. Replacement of Wire Rope :

1. Running ropes used in the main hoist, auxiliary hoist and


boom hoist excepting rotation - resistant ropes :
 Six (6) randomly distributed broken wires within one (1)
lay length.
 Three (3) broken wires in one (1) strand within one (1)
lay length.
2. Running ropes of rotation resistant construction used in the
main or auxiliary hoist :
 Four (4) randomly distributed broken wires within one (1)
by length.
 Two (2) broken wires in one strand within one (1) lay
length.
3. Standing ropes such as boom pendants :
 Three (3) broken wires within one (1) lay length.
 Two (2) broken wires at the end connection.
4. More than one third of the original diameters of the outside
wires of the strand are worn.

5. One (1) Valley break may indicate internal rope damage


requiring close inspection of this section of the rope :
 When two (2) or more valley breaks are found in one (1)
lay length the rope should be retired

6. Reduction from nominal rope diameter:


 3/64" for diameters : up to 14".
 1/36" for diameters : 7/8” - 1 1/8”.
 3/32" for diameters : 1 1/4" - 1 1/2".
Visually checked on the end connections including socket or
end attachments to running ropes, boom pendants and
other standing ropes.

5.7 WEDGE TYPE SOCKETS

5.7.1. Verify that the socket and wedge are the correct size for the
rope in use.

5.7.2. Check the socket eye & pin for roundness and wear A pin keeper
should always used with the pin.

5.7.3. Check socket and wedge for rough edges or burns

5.7.4. Cut of any welded rope end before application of socket and
wedge.

5.7.5. Wire rope clips used in conjunction with wedge sockets shall be
attached only to the unloaded (dead end) of the rope.

5.7.6. U-bolts clips shall be installed with the U-bolt section on the
dead or short end, and the saddle on the live or long End of the
rope. The spacing , torqueing and number of clips shall be in
accordance with the crane manufacturer’s specifications or
appropriate reference.

5.7.7. Ensure that the live End of the rope is on the straight side of the
socket & the dead end on the tapered side of the socket. (ASME.
B30.9 ) As for method of installing wedge socket attachment).

5.8. HOOK

5.8.1. Hooks shall be discarded where cracks exist or throat opening is 15


% more than the original throat opening. And also twisted more
than 10 degrees from the plane of the original hook can figuration.

5.8.2. Check condition of hook latch (safety latch).

5.9. SHEAVES

5.9.1. Check sheaves for :

1. Wear and corrugation.


2. Broker or chipped flange.
3. Cracks in the hub.
4. Freedom of rotation without drag.
5. Sheaves and shafts checked for bearing or shaft wear.

5.9.2. Sheave guards :

All sheaves including running block shall be provided with guards


or other suitable devices to prevent the rope from coming out of
the sheave groove.

5.10. HYDRAULIC SYSTEM

5.10.1 Visually check hydraulic pump:

1. Loose and missing bolts.


2. Leakage and damage of host.
3. Pressure of pump and compare to the manufacturers
specified pressure setting in each hydraulic circuit.

5.10.2 Visually check hydraulic oil tank for crack and damage
and also oil level.

5.11. SWING MECHANISM

5.11.1 Visually check for cracking and deformation.

5.11.2 Check for excessive wear of components and contamination by


lubricants or other foreign matter.

5.11.3 Brakes :
1. Check the parking Brake & shall be capable of remaining in
the engaged position without the attention of the operator.
2. When provided the dynamic fraction brake check the brake
to stop , hold or retard the rotation motion of the upper
structure.

5.11.4 Swing Lock :


When Provided, it shall be constructed to with stand maximum
swing torque developed by the swing mechanism ,
prevent accidental engagement or disengagement and shall be
controllable by the operator at his station.

5.12. CONTROLS, CABS & MISCELLANEOUS EQUIPMENT

5.12.1 Check the control operations and functions shall be clearly


marked and easily visible.

5.12.2 Control levers for boom hoist , load hoist , swing and boom
telescope ( when applicable ) shall return automatically to
their center (neutral) positions on release.

5.12.3 Check the condition of door, window, platforms and walkways.

5.12.4 Load charts should be provided inside of cabs.

5.12.5 Function check of the boom hoist limiter and overload


protective device and anti two block system.

5.12.6 Ballast or counter weight when applicable, crane should not be


operated without the appropriate Ballast or counter weight in
place as specified by the crane manufacturer.
5.12.7 Fire Extinguisher :
A carbon dioxide, dry chemical or equivalent fire extinguisher of
sufficient capacity shall be kept in the cab or vicinity of the
crane

5.13. EXAMINATION WITH NON DESTRUCTIVE TESTING

5.13.1 The critical components such as boom; hooks,


gantry, drum and other suspected area should be subjected to
test with Magnetic Particle or Liquid Penetrant or Ultrasonic
test.

5.13.2 Al1 wire rope test should be performed with a


hand spy cable.

5.14. TESTING

5.14.1 OPERATING TEST

Prior to load test after finish inspection each crane shall be


given an operational Test/function, to ensure crane functions
perform as intended. Operational testing should include the
following crane functional items :
1. Load hoisting & lowering mechanism.
2. Auxiliary hoisting & lowering mechanism (if any).
3. Boom hoisting & lowering mechanism.
4. Devices such as :
- Boom hoist panel.
- Prime mover start & stop.
- Helicopter warning light.
- Boom hoist limit.
- Anti two block.
- Load indicator.
- Boom angle indicator
5. All braking system.

5.14.2. LOAD TEST

5.14.2.1. Dynamic Load Test:


Prior to static test and good result on the operational test
each crane should be given a dynamic test to ensure
crane capacity.
Dynamic testing should include the following crane
capacity:
1. Hoisting and lowering load as much as 25%, 50%, 100%
of safe working load (SWL).
2. Boom hosting & lowering load as much as 25%, 50%
& 100% of safe working load (SWL)
3. Swing in range 180O with carrying load as much as
25%, 50%, 100%, of safe working load (SWL).

5.14.2.2 Static Load Test :


The purpose of the static load test to fulfill the
requirements of “ASME B.30.5 2015 and to verify that the
structure and other components are still good condition.
The static test load and test radius should be calculated
to load the crane as follows:

5.15 INSPECTION AFTER TESTING

Purpose of inspections is to verify & ensure the condition of the


crane after imposed by load test. The acceptability criteria for
this test shall be that no critical components exhibit any yielding,
buckling, or surface cracks. Special attention shall be given to
bolted and welded connection.

5.16. REPORTING

Any defects or deficiencies of the crane found either during inspection


or after completed, should be discussed with the owner/user, and if
necessary recommend corrective action.
Before inspection report and/or documents concern, issued to
government in order to issue official certificate, its necessary to
discuss with owner/user to ensure that there are no deficiencies.

1. Certificate of Inspection ( PT. SIGMA INSPEKSI INDONESIA)
2. Inspection Reports :
 Check List ( Visual Inspection )
 NDT Report
 Dimensional Check Report
 Testing Report.
3. Migas Certificate (Existing)
4. Operation Manual Book and Manufacturer Data Report.
5. Wire Rope Certificate.
6. History Card / Maintenance Record.
7. Report of Repair and Modification (if any)
8. As Built Drawing.

5.17. SHE INSTRUCTION

 Always follow Radiant SHE SOP and follow all the instruction as
stated above.
 Always use PPE such as Safety shoes, hand glove, safety glass, and
helmet.
 Watch out for slippery and falling apart.
 Watch out for the hazard around the areas. Stop the activities and
report to client and supervisor for unsafe working condition.
 Use fall protection equipment if working above 2 m. See Radiant
SHE SOP.
 Collect all waste and put it in agreed place.
 Do not expose any lubricant, oil or test fluid to the ground

6. ACCEPTANCE CRITERIA:

Acceptance Criteria of Discontinuity shall be in accordance with the


references document (point 3).
