HPI: ชัก 3 ครั้ง เวลา 3 ทุ่ม (14/06/2562) ตี 1(15/06/2562) ญาติให้ประวัติชักเกร็งครั้งละ 3 นาที หลังจาก
ชักตื่นรู้ตัว
ตี 2(15/06/2562) หลังชัก ญาติให้ประวัติเหงื่อมาก ไม่รู้สึกตัว ถามไม่ตอบ เลยนาส่งโรงพยาบาล มีไข้
แน่นหน้าอก ปวดศีรษะ
PMH: ปี 2560 โรคโพรงกระดูกตีบแคบ (L4-5 spinal stenosis)
SH: ปฏิเสธการดื่มสุราและสูบบุหรี่
ALL: ปฎิเสธการแพ้ยา

CT Brain (15/06/62) :
- scattered ill-defined non enhancing hypodensity white matter at right
temporoparietal and bilateral frontal lobe
- few small thick peripheral enhancing lesion at left basal ganglion and left high frontal
lesion
- Moving artifact. Lacunar infarction at left nucleus. No obvious intracranial hematoma
or fracture skull
MRI Brain (19/06/62) :
- Multiple small thick peripheral enhancing lesions with mild surrounding edema
scattering at gray white matter. Junction of bilateral cerebral hemispheres. Bilateral
basal ganglias and left pontocerebellar peduncle; suspected infection rather than
tumor.
- Irregular enhancement along suprasellar cistern and left Sylvain fissure; also
suspected infection.
- Subtle midline structures shifting to left sided
- Bilateral acute mastoiditis.
- Suspected bilateral maxillary, bilateral ethmoid and bilateral sphenoid sinusitis.

ในวันที่ 15/06/62 ผลตรวจ CSF ระบุว่า พบว่า มี Sugar fluid เท่ากับ 17 % Protein Fluid
เท่ากับ 310 % DTX หลัง Lumbar puncture เท่ากับ 170 mg% และหา CSF/Blood glucose ratio
ได้เท่ากับ 10% แต่ WBC count เท่ากับ 8 Cell/mm3 จากผลตรวจ CSF พบว่ามี Protein มากกว่าช่วง
ปกติ Glucose และ CSF/Blood glucose ratio ต่ากว่าช่วงปกติ เชื้อก่อโรคที่เป็นไปได้ คือ bacteria
Fungal และ tuberculosis เนื่องจากผู้ป่วยไม่ได้เป็นผู้ติดเชื้อ HIV จึงสงสัยว่าจะเป็น bacterial meningitis

จากนั้นวันที่ 18/6/5 ผลตรวจ CSF ระบุว่า พบว่า มี Sugar fluid เท่ากับ 54 % Protein Fluid
เท่ากับ 415.8 % DTX หลัง Lumbar puncture เท่ากับ 180 mg% และหา CSF/Blood glucose ratio
ได้เท่ากับ 30% WBC count เท่ากับ 150 Cell/mm3 Neutrophils เท่ากับ 43 % Lymphocyte เท่ากับ
57 % จากผลตรวจ CSF พบว่ามี Protein มากกว่าช่วงปกติ Glucose และ CSF/Blood glucose ratio ต่า
กว่าช่วงปกติ WBC count อยู่ในช่วง 50-500 Cell/mm3 พบ Cell differential แบบไม่จาเพาะ ได้แก่
Neutrophils และ Lymphocyte จึงเข้าเกณฑ์วินิจฉัยว่าติดเชื้อ Tuberculosis ตาม Pharmacotherapy:
A Pathophysiologic Approach Edition ที่ 11 นอกจากนี้ตามเกณฑ์วินิจฉัยตามแนวทางการควบคุมวัณ
โรคประเทศไทย พ.ศ. 2561 ระบุเกณฑ์วินิจฉัยดังนี้
เกณฑ์การวินิจฉัยวัณโรคนอกปอด1
(1) มีลักษณะทางคลินิกและผลการตรวจทางห้องปฏิบัติการเข้าได้กับวัณโรคนอกปอด เช่น น้าจากใน
ช่องเยื่อหุ้มปอดเป็น exudate และมี lymphocyte เด่น ร่วมกับมีค่า ADA (adenosine deaminase) สูง
ตามเกณฑ์ (ค่าที่เยื่อหุ้มสมองมากกว่า 20 U/L/min) หรือ AFB smear สิ่งส่งตรวจผลเป็นบวก หรือ
(2) มีผลการตรวจทางพยาธิวิทยาเข้าได้กับวัณโรค หรือ
(3) มีผลการเพาะเลี้ยงเชื้อจากสิ่งส่งตรวจผลเป็นบวก

จากผล Gram stain ของ CSF และ AFB stain ไม่พบเชื้อ เนื่องจากโอกาสผลเป็นบวกน้อย ส่วนผล
การเพาะเชื้อใช้เวลานานถึง 8 สัปดาห์ถึงจะทราบผล ดังนั้นจึงอาศัยลักษณะทางคลินิกและผลการตรวจทาง
ห้องปฏิบัติการในการวินิจฉัย จากผลตรวจทางห้องปฏิบัติการของผู้ป่วย พบว่า ลักษณะของ CSF ของผู้ป่วยมี
ลักษณะใสในวันที่ 15/06/62 ต่อมาวันที่ 18/06/62 มีลักษณะขุ่นเล็กน้อย มีจานวน Neutrophils 29% และ
Lymphocyte 71% (ซึ่งถือว่าเป็น mononuclear cell มากกว่า 95%) Protein 415.8 mg% (สูงกว่าค่า
ปกติ) และ glucose 54 mg% (ต่ากว่าระดับกลูโคสในเลือด) WBC count 150 cell/mm3(อยู่ในช่วง 100-
500 cell/mm3) จึงเข้าได้กับเกณฑ์วินิจฉัยว่าเป็นวัณโรคที่เยื่อหุ้มสมอง ตามแนวทางการควบคุมวัณโรค
ประเทศไทย พ.ศ. 2561
ปัจจัยเสี่ยงที่จะติดเชื้อวัณโรค ได้แก่
1.ประชากรที่เป็นกลุ่มเสี่ยงในการสัมผัสและติดเชื้อวัณโรคเช่น ผู้ที่อยู่ใกล้ชิดกับผู้ป่วยวัณโรค
(contacts) ผู้ต้องขัง (prisoners) บุคลากรสาธารณสุข (health care worker) แรงงาน ข้ามชาติ (migrant
labors) ผู้สูงอายุและเด็กผู้ที่อาศัยอยู่ในชุมชนแออัดค่ายอพยพหรือสถานสงเคราะห์ เป็นต้น
2.ผู้ป่วยที่มีโรคร่วม เช่น ผู้ติดเชื้อเอชไอวี (HIV) ผู้ป่วยเบาหวาน (DM) ผู้ที่ได้รับยากดภูมิคุ้มกัน ผู้ป่วย
โรคปอด อุดกั้นเรื้อรัง (COPD) ผู้ป่วยโรคฝุ่นหิน (silicosis) และผู้ป่วยโรคไต
3) ผู้ที่ภูมิต้านทานที่ชั้นเยื่อบุ (mucosa) ของหลอดลมและเนื้อเยื่อที่ปอดมีการเปลี่ยนแปลง จากโรค
ฝุ่นหิน การสูดควันบุหรี่ เป็นต้น
ในผู้ป่วยรายนี้ไม่มีโรคร่วม ไม่สูบบุหรี่ และให้ข้อมูลว่าไม่เคยติดเชื้อวัณโรคมาก่อน และไม่เคยอยู่
ใกล้ชิดกับผู้ป่วยวัณโรค จึงอาจเป็นไปได้ว่าผู้ป่วยเคยใกล้ชิดกับผู้ป่วยวัณโรคแต่ไม่ทราบว่าเป็น

การรักษา
ผู้ป่วยยังควรได้รับยารักษาวัณโรคต่อไป ในการเลือกยาสาหรับผู้ป่วยรายนี้เนื่องจากผู้ป่วยไม่เคยมี
ประวัติเป็นวัณโรคมาก่อนและไม่เคยได้รับยารักษาวัณโรค จึงควรได้เป็นสูตรเดิมที่ได้รับตอนเป็น Empiric
treatment ซึง่ ยังไวต่อยาสูตรนี้อยู่ ตามแนวทางการรักษาวัณโรค Official American Thoracic
Society/Centers for Disease Control and Prevention/Infectious Diseases Society of America
Clinical Practice Guidelines: Treatment of Drug-Susceptible Tuberculosis ในการรักษา
tuberculous meningitis18 แนะนาให้เริ่ม Isoniazid Rifampicin Pyrazinamide และ Ethambutol นาน
2 เดือน จากนั้นให้ ให้ Isoniazid Rifampicin ต่ออีก 7–10 เดือน ซึ่งสอดคล้องกับแนวทางการควบคุมวัณโรค
ประเทศไทย พ.ศ. 2561ที่ระบุว่าระยะเวลาของการรักษาวัณโรคเยื้อหุ้มสมองอักเสบและวัณโรคสมองต้อง
มากกว่า 12 เดือน ดังนั้นระยะเวลาในการรักษาในผู้ป่วยรายนี้ถือว่าเหมาะสม

Guidelines: Treatment of Drug-Susceptible Tuberculosis แนะนาให้ corticosteroid นาน 6-8 สัปดาห์
แนวทางการให้ Corticosteroid ตาม Leonard JM19 แนะนาขนาดยาคือที่Dexamethasone ที่สัปดาห์ที่1
และ2 ให้ 0.3-0.4 mg/kg/day จากนั้นสัปดาห์ที่ 3 ให้ 0.2 mg/kg/day สัปดาห์ที่ 4 0.1 mg/kg/day สัปดาห์
ที่ 5 ให้ 4 mg/day จากนั้นลดลงสัปดาห์ละ 1 mg/day ระยะเวลารวมทั้งสิ้น 8 สัปดาห์ กรณีให้Prednisone
แทนDexamethasone ที่สัปดาห์ที่1และ2 ให้ขนาด 60 mg/day จากนั้นลดลงสัปดาห์ละ 10 mg/day รวม
ทั้งสิ้น 8 สัปดาห์ จากข้อมูลการให้ยาของผู้ป่วย เนื่องจากผู้ป่วยใส่ท่อช่วยหายใจ corticosteroid ชนิดฉีดจึง
เหมาะสมมากกว่ า ชนิ ด รั บ ประทาน ดั ง นั้ น ผู้ ป่ ว ยรายนี้ จึ ง ควรได้ Dexamethasone พบว่ า ตั้ ง แต่ วั น ที่
17/06/62-23/06/62 (สัปดาห์ที่ 1) ได้ Dexamethasone 12 mg IV q 12 hr (24mg/day) ได้ ตั้งแต่วันที่
24/06/62-26/06/62 (สัปดาห์ที่ 2) Dexamethasone 9 mg IV q 12 hr (18 mg/day) และเนื่องจากผู้ป่วย
สามารถรับประทานได้ จึงต้องเปลี่ยนจากรูปแบบฉีดเป็นรูปแบบกิน เนื่องจาก Dexamethasone ไม่มีรูปแบบ
ยากิ น ดั ง นั้ น จึ ง ให้ เ ป็ น Prednisone แทน โดย Prednisolone 5 mg มี ผ ลต้ า นการอั ก เสบเที ย บเท่ า กั บ
Dexamethasone 0.75 mg ดังนั้นวันที่ 26-30/06/62 (สัปดาห์ที่ 2) ผู้ป่วยได้ Prednisolone (5) 6X2 po
pc (เทียบเท่ากับ Dexamethasone 18 mg/day) วันที่ 1-7/07/62(สัปดาห์ที่ 3) ผู้ป่วยได้ Prednisolone
(5) 5X2 po pc วั น ที่ 8-14/07/62 (สั ป ดาห์ ที่ 4) ผู้ ป่ ว ยได้ Prednisolone (5) 3X2 po pc วั น ที่ 15-
21/07/62 (สัปดาห์ที่ 5) ผู้ป่วยได้ Prednisolone (5) 2X2 po pc วันที่ 22-28/07/62(สัปดาห์ที่ 6) ผู้ป่วยได้
Prednisolone (5) 3x1 po pc วันที่ 29/07/62-4/08/62 (สัปดาห์ที่ 7) ผู้ป่วยได้ Prednisolone (5) 2x1
po pc วันที่5-11/08/62 (สัปดาห์ที่ 8) ผู้ป่วยได้ Prednisolone (5) 1x1 po pc ถือว่าเหมาะสม
นอกจากนี้ผู้ป่วยมีประวัติชักก่อนมาโรงพยาบาล 3 ครั้ง คือเวลา 3 ทุ่ม ตี1 และตี2 นานครั้งละ 3 นาที
ตามแนวทางเวชปฏิบัติโรคลมชักสาหรับแพทย์ พ.ศ. 25596 แนะนาว่ากรณีเกิดการชักครั้งแรก อาจพิจารณา
เริ่มยากันชักตั้งแต่การชักครั้งแรก ในผู้ป่วยที่ตรวจร่างกายพบความผิดปกติทางระบบประสาท และเป็นการชัก
ที่ไม่มีปัจจัยกระตุ้น โดยเฉพาะกลุ่มผู้ป่วยที่เคยมีประวัติโรคหรือความผิดปกติทางสมองมาก่อน เช่น พบความ
ผิดปกติจาการตรวจรังสีวินิจฉัย(CT scan หรือ MRI brain) การติดเชื้อในสมอง การเลือกยากันชักในผู้ป่วยราย
นี้ พบว่ า การเลื อ กยาให้ Phenytoin ในการรั ก ษาถื อ ว่ า ไม่ เ หมาะสม เนื่ อ งจาก Phenytoin เกิ ด drug
interaction กั บ ยารั ก ษาวั ณ โรค ได้ แ ก่ isoniazid และ rifampicin ซึ่ ง มี ผ ลทั้ ง ลดและเพิ่ ม การก าจั ด ยา
ตามลาดับ เป็นผลให้ระดับยาในเลือดคาดการณ์ยาก (นอกจากนี้การให้ maintenance dose เป็น Dilantin
100 mg +NSS 100 mL IV drip q 8 hr จากนั้นเปลี่ยนเป็นรูปแบบรับประทาน โดยให้ Dilantin (50) 2X3
po pc โดยการบดเพื่อ feeding เนื่องจากผู้ป่วยใช้ท่อช่วยหายใจ ซึ่ง Dilantin ความแรง 50 mg (Infatab)
เป็นรูปแบบที่สามารถบดแล้ว feed ให้ผู้ป่วยได้ แต่การให้ infatab ในกรณีผู้ป่วยกลับบ้าน ถือว่าไม่เหมาะสม
เนื่ อ งจาก จ านวนเม็ ด และระยะเวลาในการรั บ ประทานหลายครั้ ง ต่ อ วั น มี โ อกาสที่ ผู้ ป่ ว ยเสี่ ย ง non
compliance ทาให้ ร ะดับ ยาไม่อยู่ ในช่ว งที่ส ามารถคุมชั กได้ อาจทาให้ อาการชักกาเริบ ได้ จึงควรได้เป็ น
Dilantinรูปแบบ capsule ความแรง 100 mg ซึ่งเป็น Extended release รับประทาน 3x1 hs ซึ่งรับประทาน
วันละ 1 ครั้ง ซึ่งช่วยเพิ่มความร่วมมือในการรับประทานยาได้ แต่อย่างไรก็ตามในผู้ป่วยรายนี้การเลือกยา

6.แนะนาให้ผู้ป่วยกินยารักษาวัณโรคอย่างสม่าเสมอ เพื่อป้องกันการดื้อยา
7.แนะนาให้รับประทานยากันชักอย่างสม่าเสมอ เพื่อป้องกันการชักกาเริบ
Future plan
ประเมินผลการรักษา และอาการไม่พึงประสงค์ ติดตามค่า Liver function test และ Renal
Function test หากมีค่าที่ผิดปกติอาจพิจารณาปรับเปลี่ยนสูตรยา
หากผู้ป่วยเกิดอาการแพ้ยาอย่างรุนแรง อาทิเช่น Steven Johnson syndrome หรือ toxic
epidermal necrolysis พิจารณาหยุดยาทันทีและปรับเปลี่ยนสูตรยารักษา
หากผู้ป่วยรับประทานยารักษาวัณโรคแล้วเกิดการดื้อยา isoniazid พิจารณาเพิ่มขนาด rifampicin
และให้ยากลุ่ม fluoroquinolone ร่วมด้วย หรือให้ aminoglycoside ร่วมด้วยก็ได้ นอกจากนี้ยังมีคาแนะนา
ให้เพิ่มระยะเวลาในการรักษาเป็น 18-24 เดือน ขึ้นกับความรุนแรงของโรค อัตราการตอบสนองต่อการรักษา
และภูมิคุ้มกันต่อยา28
การพิจารณาหยุดยากันชักเมื่อผู้ป่วยไม่มีอาการชักมาอย่างน้อย 2 ปี และไม่มีความผิดปกติของการ
ตรวจร่างกายทางระบบประสาท หรือจากการตรวจทางเอกซเรย์คอมพิวเตอร์ หรือคลื่นแม่เหล็กไฟฟ้าสมอง
โดยจะค่อยๆลดขนาดยาที่กินจนหมด นอกจากนี้กรณีผู้ป่วยเกิดการแพ้ยา levetiracetam พิจารณาหยุดยา
และเปลี่ยนเป็นยาตัวใหม่ก่อน 2 ปี

Objective data: V/S: BP 171/125 mmHg
Assessment:
Hypertension หรือโรคความดันโลหิตสูง คานิยามตามแนวทางการรักษาโรคความดันโลหิตสูง ใน
เวชปฏิบัติทั่วไป พ.ศ. 25627 หมายถึง มี systolic blood pressure (SBP) > 140 มม.ปรอท และ/หรือ
diastolic blood pressure (DBP) >90 มม. ปรอท โดยอ้างอิงการวัดความดันโลหิตที่สถานพยาบาล
ลักษณะอาการทางคลินิก มักไม่ค่อยมีอาการ หรืออาจพบว่ามีอาการปวดศีรษะ มึนงง เวียนศีรษะ
ความดันโลหิตสูง อาจทาให้เกิดภาวะแทรกซ้อนได้ 2 กรณี ได้แก่
กรณีที่ 1 ภาวะแทรกซ้อนจากความดันโลหิตสูงโดยตรง ได้แก่ ภาวะหัวใจวายหรือหลอดเลือดในสมอง
แตก
กรณีที่ 2 ภาวะแทรกซ้อนจากหลอดเลือดแดงตีบหรือตัน เช่น กล้ามเนื้อหัวใจขาดเลือดเฉียบพลัน
หรือเรื้อรัง ทาให้หัวใจเต้นผิดจังหวะ อาจจะทาให้ถึงแก่ชีวิตได้ หลอดเลือดสมองตีบ เกิดอัมพฤกษ์ อัมพาต
หรือหลอดเลือดแดงในไตตีบมากถึงขั้นไตวายเรื้อรังได้
การจาแนกความรุนแรงตามแนวทางการรักษาโรคความดันโลหิตสูง ในเวชปฏิบัติทั่วไป พ.ศ. 2562

ถ้ายังไม่ได้ผลอีก อาจพิจารณาให้ยาติดต่อกัน 6 สัปดาห์
