Dear Sir,

Thank you for giving us the opportunity to offer our services to your company.
Based on your request regarding the quotation similar for the cable laying operations of
PLDT submarine Cable, I provide an almost total package of works based on your
request.

We are confident that with our good knowledge of the area and with the proper
experience, understanding of project similar to your needs, we can perform the job well
and be able to offer our best quotation, justifiable and fair to the works stated. Our
company had been principally involved in the same job situation as we have located,
surveyed, retrieved and relayed the submarine cable of the (GUIMELCO) Guimaras
Electric Co of Guimaras Island Province. Working at a maximum of 60 meters deep
between Panay Iloilo and Guimaras Island, we have successfully recovered the submarine
cable for the repair of their Engineers and relayed back to sea floor as instructed. We have
worked also in the major repair and construction of cable sinkers of (AKELCO) Aklan
Electric Co Submarine Cable laid at Caticlan to Boracay Island channel, as well as that of
PLDT submarine cable in various areas of the Visayas Region. Our Commercial
Divers are all professionally trained and licensed to conduct commercial underwater
works. We have professional and technical equipments for the job, such as Sonar
Underwater Scanning Device with direct built in (GPSS) Global Positioning Satellite
System, Our Underwater Closed Circuit T/V (CCTV) is capable of monitoring
underwater works and video footage which can be recorded on CD.

A Mobilization and Demobilization of P50,000.00 (Fifty Thousand Pesos) is required

for Aklan area only.

ENVIRONMENTAL COMPLIANCE SURVEY, REPORT AND DATA

PRESENTATION FOR DENR APPLICATION:

P150,000.00 total survey package. Diving survey using Transect Line for marine
life species documentation, using Quadrant for marine life and other
benthic life form population count. Underwater Video/Photo for
underwater picture documentation and analysis of data and Technical
Report presentation. (CASH DEPOSITED ALREADY & DIVING
SURVEY EMPLEMENTED)

FABRICATION OF CONCRETE ARMOR: (DISCOUNTED COST)

P3,450.00 per unit set of 8’x 8” dia. Fabricated on site. Concrete class mixture is
Class A (1 : 2 : 4). (Please see technical drawing) X 1148 pcs armor =
3,960.600.00 pesos Three million, nine hundred sixty thousand, six
hundred pesos.

EXCAVATION, CONSTRUCTION OF TERMINATION POINT:

P40,000.00 per station for 3m x 3m x .10m wide top concrete pavement. With
1.5m dia. X 2m depth circular termination room, waterproof sealed wall X
two units (Tabon & Tambisaan) = 80,000.00 Eigthy thousand pesos

LAYING-OUT AT SEA, ARMOR APPLICATION AND EXCAVATION, BURIAL

OF FOC CABLE AT BEACH AREA TERMINATION POINT. (DISCOUNTED
COST)

P1,400.00 per linear meter of cable laid out. Using Tugboat and Barge or any
similar vessel safe for the operation. Professional scuba divers to work
safely and efficiently underwater. X 2,800 m = 3,920,000.00 (Three
million, nine hundred twenty thousand pesos)

NOTE: ALL PRESENTED COST OF WORKS ARE REQUIRED AND SUBJECT

TO 12% VAT (a government requirement).
OUR SCHEDULE: (manpower, vessels and facilities)

Per our estimate and considering the situation that is off shore we can only quote
to the best of our experience and knowledge in the area limited to the acceptable,
prevailing working condition based on safety consideration of the workers and divers and
that of the vessels and facilities employed.

Per our understanding we expect that this job is under urgent category matter of
your company and that our services if, employed is on a continuing manner until
completion (unless otherwise terminated with reasonable cause). Such expectations and
arrangement is vital as we include Mobilization and Demobilization factor in our
quotation as well as our workforce scheduling of our performance.

Based on standard requirements, the cable will be buried to a depth of 1.0 meters
at the base of the Termination Point. Excavation and installation procedures will be done
with care not to damage the cable. This contractor shall employ the best effective and
safe handling procedure for the said communication fiber optic cable.

Manpower schedule: we can employ the required no. of technical divers and
man power needed by the project to meet the deadline comfortably provided financial
obligation is met by the project.

Vessel schedule: we can provide the appropriate sea craft and vehicles needed by
the project for safe conduct of the Laying process.

Equipments and other support facilities: we have the necessary equipment for
the job and if needed we have extensive tie ups with other heavy equipment owners and
vessels in the area.

OUR REPORT:

We submit, present to your company technical report on print, including all the
photo documentation on Video clips on a CD Data format. A file copy of this report shall
be submitted in P.C. data format for your computer files analysis and possible data
processing, together with prints and photo reports.
BILLING REQUIREMENTS:

BILLING REQUIREMENTS:

After complying all the necessary requirements, our company requires a

deposit of 100% of the total mobilization and demobilization cost, the
Environmental Assessments Diving Survey cost, the Termination station
construction cost. And the Fabrication of number of Concrete Armor.

A 50 percent of the total computed cost of the total linear agreed on the Layout
and Concrete Armor Installation cost. The said deposit shall form part of the “Permit to
Proceed Notice” of the project, and must be deposited in our bank account ready for
disbursement.

Progress billing of 50% of the remaining payables shall be requested after

confirmed 50% of Layout and Concrete Armor Installation works completed. And final
payment upon final acceptance of the completed project and submission of reports.

WARRANTY OF WORKS:

We offer a workmanship warranty that all agreed work specifications will be

followed and that any defect noted particular only on the contracted service will be
repaired or replaced at our expense, provided however no other third party involve or
directly or indirectly cause the damage or defect within 1 year upon installation.
All and any damages cause by force majeure such as typhoon, earthquake and other
nature calamities are not included.

RETENTION:
In EXCAVATION, CONSTRUCTION OF TERMINATION POINT and LAYING-OUT
AT SEA, ARMOR APPLICATION AND EXCAVATION, BURIAL OF FOC CABLE AT
BEACH AREA TERMINATION POINT we agree that after acceptance and delivery,
COMTEL shall retain 10% of the total amount of the contract final billing and will hold
this amount for a period of (3) three months as guarantee for the work conducted by the
SCUBATECH.

After the said period, SCUBATECH may opt to post a retention bond from an insurance
firm that is equal to the amount of the retention monies for a period of one year.

Upon posting of the said bond, SCUBATECH can withdraw the retention monies from
Comtel.
MOBILIZATION:

Our company requires 5 – 7 days preparation time upon receipt of the complete
notice Permit To Proceed to mobilized and be on the job site

VALIDITY OF OFFER:

Due to some unstable costing factor affecting the cost and schedules such as Fuel
and others, our cost offer is valid within 30 days after which a confirmation is needed.
The Contractor shall coordinate closely with your company management, particularly on
safety and company policies.

I, Engr ALLAN E. BEJENO, representing COMTEL INDUSTRIAL SERVICES Inc,

duly authorized by its President Engr. Romulo Q. Requierme, do hereby express our
conformity with the above proposal from SCUBATECH for Boracay/Caticlan dated
February 8, 2013 and do hereby signify our intent to permit SCUBATECH to proceed on
the works stated in the said proposal.
