We, ESPERANZA QUISMONDO and LORETA FERRER, both of legal age, Filipinos and residents
of Canaoalan, Binmaley, after having been duly sworn to in accordance with law, hereby depose
and say

That we know personally late EUFROCINA PALOMAR MARQUEZ, a resident of Canaoalan,

Binmaley , Pangasinan;
That as per Death Certificate her name appeared EUFROCINA PALOMAR MARQUEZ;
That as per tax Declaration No. 983 and 984 her name appeared CIPRIANA PALOMAR instead of
EUFROCINA PALOMAR MARQUEZ;
That the name EUFROCINA PALOMAR MARQUEZ and CIPRIANA PALOMAR is refers to one the
same person;
That we know the facts of the foregoing because we have been immediate neighbors and closed
family friend to the present;
That we are executing this affidavit to attest to the truth of the foregoing statements and for
legal intent and purposes, it may serve.
