a. Utilities industries have an average of 700 staff per year. Determine


the Fatality Rate for year 2019.

b. The total number of hours work for Construction is 250,000. Calculate


the number of occupational illness, if there was no fatality case
reported, and the number of illness and injury is similar in OSHA
Incidence Rate.

c. Calculate the Lost Time Injury Rate of Transport & warehouse, if in the
of OSHA Incidence Rate reported has total lost workdays of 19.

2. The Three process units are in a plant. The units have FARs of 0.6, 0.6 &
2.8 respectively in a year.

a. Calculate the overall FAR for this plant, assuming worker exposure to
all three units simultaneously?

b. Assume now that the units are far enough apart that an accident in one
would not affect the workers in another. If a worker spends 35% of his
time in process area 1, 55% in process area 2 and 10% in process
area 3 in a year, evaluate his overall FAR?

3. The following data is accident description of Aspen Sdn Bhd company that
has various of activities for year 2018

when propane fumes in the tank exploded. Two employees had a third
degree burnt and died a week later.

May 231 Pak Leman has been working in rice processing plant for over 15 years. He
was complaining about having severe chest pain and bad cough. He was
sent to the hospital for thorough examination. The doctor informed that he
has severe lung disease due to inhalation of padi husk dust for long period
of time. He was given 60 days of rest.

May 225 Alias was working at shelf. The 200kg gunny sack of padi fell in front of
him and caused loses about 200kg grain.

May 230 Ng was performing decking work at a height of 10 feet above ground
level and fell through the decking caused twisted his ankle.

June 211 No incident was reported

July 211 No incident was reported

August 212 Shima was performing housekeeping job in the detergent store and found
fainted by his coworker; was rushed to clinic and work as usual on the
following day.

August 210 Sega was installing heating and cooling equipment and fell from a ladder.
An orthopedic confirmed joint dislocated and advised him to rest for at
least 30 days

Oct 211 2 workers were hospitalized for 2 weeks after consuming dinner provided
by the company caterer suspected due to leptospirosis.

Dec 211 Arafiah is one of the cleaners in furniture manufacturing company that using
rengas wood. She has been working for the last 4 years. The safety
executive noticed that she has serious skin disease on her left hand; was
rushed to clinic and work as usual on the following day
