MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS;

This agreement is entered into on this ________ day of _____________________ 2017 in the Municipality of
Naga City, Camarines Sur. By and between:

The Bermudez Center for Arts and Technology, Inc. (BCAT) a non-stock and non-pront Catholic Church based
Social development Non-Government Organization, organized under laws of the Philippines with office address at Brgy.
San Juan San Lorenzo Minalabac, Camarines Sur,4414 Philippines herein represent by its School President, Gerry A.
Bermudez, RN, MPA., herein after referred to as the FIRST PARTY;

And

_______________________ A practicing physician has a common stand and aim at promoting and propagating
the health benefits of all trainees that is enrolled in all Technical/Vocational Courses offering of the Bermudez Center for
Arts and Technology, Inc.

NOW THEREFORE, the above parties hereby enter into this agreement as follows:

1. The first party agrees to pay the Health Service rendered to all the Enrolled Trainees.

2. The second party agrees to render Health Service as specified above, for the common health benefits of all
trainees enrolled in Technical/Vocational Courses.

3. Both have consented that this MOA will take effect upon signing and shall remain valid until December 2025.
Unless, terminated earlier upon written notice either of the parties..
