Parental Permission for Student Participation in Research

Title: Smartphone as a Learning Tool: Its Effect on the Academic Performance of the
Senior High School Students

The purpose of this form is to provide (as the parent of a prospective research study
participant) information that may affect your decision as to whether or not let your child
participate in this research study. Kindly read the information below before deciding whether or
not to give your permission for your child to take part in this study. If you decide to let your
child be involved in this study, this form will be used to record your permission.
If you agree, your child will be asked to participate in a research study about the “Effect
of smartphone on the academic performances of the Senior High School Students” when used as
a Learning Tool. The purpose of this study is toexplore the significant relationship of the level of
the effect of using smartphone as a learning tool, determine the better effects of smartphone as a
learning tool on the respondents, determine the level of effect of using smartphone as a learning
tool and determine the level of the students’ academic performance among Senior High School.
If you allow your child to participate in this study, they will be asked to check the boxes
that correspond to their answer in the questionnaire. This study will be conducted during vacant
time to avoid interruption of classes. This will take only for one hour. There will be 50
respondents in the study.
NOTE: There is no foreseeable risk in participating in this study yet these will help the
researchers find the information to complete the study.
