Project Title:

“Rehabilitation of Water System”

Description:

The project primarily intend is ensure availability of safe drinking water in every
household in Baranggay Camam-onan. and to eliminate the incidence of water-
borne diseases in the Barangay Camam-onan which was recorded at 30 cases of
skin diseases during the preceding year. The said barangay have their water
connections provided by the Municipality of Gigaquit. It will also augment LGU’s
undertaking in improving the water system of the baranggay, the LGU and DENR
and other partnership organizations will be in charge in ensuring the sound
implementation of the project and a direct partnership with the barangay Council
and its youth will also needed during the construction phase that will contribute
tolabor components of the project construction.

Objectives:

• Ensure availability of safe drinking water in every household in Barangay

Camam-onan.

• Aim to provide the 465 individuals in 96households in the barangay an access of

safe drinking water.

• Promote good health and help prevent from being afflicted by disease caused by
contaminated water.

In general, the project aimed at reducing water-borne diseases of the barangay. It

will promote good health and will ensure the availability of safe drinking water in
every household in the said barangay.

Once the project is completed it will be turn over to the said Barangay and officials shall
be the over-all in-charge to its operation and maintenance, also collection of fees shall be
imposed ate the rate of 20.00 per household to cover the operation expense

Key outcomes by the end of the year 2020- 2021

By the end of 2021 the rehabilitation of water system is successfully implemented

achieving its goal on sustaining clean water and sanitation.

Learning Objectives:

• Educate people through symposium about water scarce resource and give
awareness about the relationship between water, proper sanitation, and to our
health also its corresponding prevention to the possible risk due to water
contamination.

Evaluation:
 This project will not only help the community from their problem about clean
water but also it will prevent them from any infections and communicative
diseases. On evaluating this project survey will be conducted by DENR and LGU
expertise to ensure functionality and its safety use.

Project title: Free Check-up and Consultation

Description:

The Free Check-up are set up with a sacred aim to bring awareness amongst the deprived
population of the Barangay Bonifacio, Surigao City who have no access to basic healthcare
services or knowledge about the diseases they are suffering from. In this barangay there are some
residents lost their lives and have a serious diseases because they don’t have proper check-up and
consultation to a Physician.  So, Free Check-up provides free medical advice, medicine to the
unfortunate people and refers for specialized treatment. They need to attend these to get free
check-up and treatment.

Objectives:

The objective of conducting the health is to create awareness of general health is to create
awareness of general health among the people, provide general health care services and
counselling them on basic healthcare and hygiene.

To build a replicable model of healthcare in the Barangay Bonifacio, Surigao City,

ensuring all people in the target area has access to quality affordable healthcare.

Once the project is completed and the cases of diseases on this barangay will be lessen
this group will only give some supply to the barangay health center, and the continue
monitoring in every households in span of 5 months.

By the end of 2021 every households in this barangay has knowledge on their local
diseases and have idea how to avoid those diseases.

Learning Objectives:

We are conducted by health professionals to carry out a limited health intervention

amongst the underprivileged community. The poor attend these to get free check-up and
treatment. Getting the appropriate kind of health checkup is vital for every human being and
while considering it, some important factors like age, lifestyle, family background, and risks are
taken into account.

Health examinations and tests at the early stages of the illness can help to cure it faster
and save a life before it can cause any damage. One can live longer and healthier only when the
individual gets the right kind of health check-up, screening, and treatments. Even the most basic
checkups can identify underlying illnesses.

Teaching-Learning Strategy:

Evaluation:

This project will not only help the community from their problem about their
diseases and illness but they will also learn how to avoid those diseases and be
healthy. On evaluating this project survey will be conducted by DOH and
Barangay Health Center expertise to ensure how and what are the other needs of
the community.
