AFFIDAVIT OF CORRECTION OF GENDER

I, ANGEL VIC A. SALVANIA, of legal age, Filipino, single, and a

resident of Matina, Davao City, after having been sworn in accordance with
law, hereby depose and say that:
 
1. I was born on 27 April 1990 at Tunggol, Pagalungan,
Maguindanao, and my Certificate of Live Birth was registered with
the Local Civil Registrar of the municipality of Pagalungan,
Maguindanao.

2. When I secured a copy of my Certificate of Live Birth from the

National Statistics Authority (NSA), Davao City, I found out that
the entry for my Sex/Gender was erroneously indicated as MALE
when in truth and in fact I am a FEMALE, as reflected in my
Medical Certificate issued by the Southern Philippines Medical
Center, herein attached and to form part of this affidavit.

3. I hereby affirm and confirm that my gender is a FEMALE and not

a MALE as erroneously indicated in my Certificate of Live Birth.

4. I am executing this affidavit to affirm and confirm the foregoing

facts and for whatever legal purpose this may serve best.
