AFFIDAVIT OF STATEMENT OF FACT

I, Julie Morana Barbonio, of legal age, Filipino, and a resident of 7157 M.

Ocampo St, Pio Del Pilar, Makati City, after having been duly sworn to in accordance
with law, hereby depose and state, that:

1. I am the mother of the following children with their pertinent details as

follows:

b) Prime Jethro Barbonio Labasan 01 August 2015 "B"

c) Julian Paul Barbonio 21 December 2016 "C"

2. My first two (2) children Precious Jul Barbonio Labasan and Prime Jethro
Barbonio Labasan were duly acknowledged by their father Primo G. Labasan as shown in
the affidavit of acknowledgment/admission of paternity at the dorsal portions of their
birth certificates (Annexes "A" and "B");

3. On 21 December 2016, I gave birth to my third child Julian Paul Barbonio

(Annex "C") whose father cannot be located and his whereabouts is currently unknown;

4. At the time of the birth of my three (3) children I am not legally married;

5. That there was an error in the birth certificate of my second child Prime
Jethro Barbonio Labasan when his birth order was indicated as "THIRD" when it should
only appear as "SECOND" based on the sequence and date of births of my three (3)
children;

6. I am executing this affidavit to attest to the truth of the foregoing facts

and for whatever legal purpose it may serve and to secure all the benefits provided
under the law for SSS, Philhealth, insurance, maternity benefits, etc.
