Minutes of Meeting
1ST QUARTER PTA MEETING
A meeting of the persons involved in the 1ST QAURTER PTA Meeting of Lasip National High School, Lasip,
Lingayen, Pangasinan was held on February 01, 2019; 2:00 P. M. at the G-12 C2B Room.

Call to Order:
School Principal, Head Teachers, Class adviser and concerned teachers from Grades 7 to 12 called the meeting to
order at 2:00 P. M. A quorum of committee members headed by school head was present and the meeting having
been duly convened and was ready to proceed with the agenda.

Coordinator/ Chairman Report:


“All study, no socialization makes a student a dull one”, quoted by our School Principal-Madam Lucy A. Nicomedez
who delivered enlightening opening remarks. In line with, head teachers and concerned class advisers presented
the agenda and welcomed everyone to the meeting about the Voluntary Promenade of Grades 9-12 students.
Afterwards, Madam Erylene L. de Guzman-JHS, Master Teacher I, gave proposal of the elective promenade’s
venue, food and beverages, dress code and others. A number of questions were asked and extensive discussion
ensued.

Status Update:
With the proposal introduced, it is wholeheartedly agreed by majority of parents, students and teachers that:
a) Star Monica, Grand Pavilion, Lingayen, Pangasinan will be the venue of the promenade,
b) A Php. 400.00 per students will be collected, inclusive of the following:
1. Venue rental including air con, sound system, lights and assistance service,
2. Foods for dinner including meat, vegetables, dessert, rice, sodas and drinks,
3. Invitations, certificates, sashes, gate pass tickets and other communication letters, and
4. Roses and tokens.
c) It will be held on February 22, 2019 (Friday), 6:00 PM,
d) The dress code for the said event will be:
1. Grade 9 – Red
2. Grade 10 – Blue
3. Grade 11-12 – Black
(Note: Dress for ladies and Long Sleeve for gents)
e) The payments should be collected within the period of February 15 until February 20, 2019.

Approval of the Plans:


Mr. Pepito R. Baniqued, Jr, presented to the School Principal, Head Teachers, concerned class adviser and
parents the minutes of the meeting for approval, whereupon motion duly made, seconded and unanimously
adopted, the minutes were approved as presented.

Adjournment:
There being no further business to come before the meeting, the meeting was adjourned at 4:30 P. M.
