ACCOMPLISHMENT REPORT
Title of the Activity: School 5th General Assembly
School Year 2021-2022

Training/Activity Objectives:

The purpose of the activities is to:

 Discuss the development of the implementation of face to face classes
 Inform the parents/guardians on the importance of COVID-19 vaccination to children
 Update parents/guardians on the accomplishment of the school for the quarter

Introduction:

Arsenio Santos Memorial Elementary School conducted its 5th General Assembly last June
22,2022 at 9:00 a.m. to 11:00 a.m. held at the School Covered Court. 55.49% or 389 parents/guardians
attended the said event and those 44.51% or 312 guardians who weren’t able to attend were oriented and
given notes by the adviser during the retrieval of modules.

Narrative Report:

The assembly started through a prayer led by Ms. Zenaida L. Bernardino, followed by the
checking of attendance.
Mrs. Ruth R. Santos provided comprehensive discussion on the development of the
implementation of Face to Face Classes. Progress of learners attending in-person classes was
revealed to motivate other parents to allow their children to capture the benefits of Face to Face
Classes.
The importance of Covid-19 vaccination was tackled by Mrs. Maricel R. Salazar to convince
parents to get their children vaccinated. The benefits of vaccination were enumerated to help
parents realize the mere value of vaccines.
To give updated on school’s accomplishment, Mrs. Salazar cited the various achievements of
the school for the quarter.
The meeting ended with proper guidelines and ideas for the teachers and parents to follow
and prepare.

Attached to this is the documentation of the meeting with the teachers, parents and the
Officer in-Charge.
