(2) Corrective actions included removing loose concrete and reinspecting forms prior to pouring concrete. (3) The root cause was determined to be non-compliance with structural calculations for tie rod spacing, which caused the form collapse.     Disposition: For Compliance

    Corrective Action Taken: After removal of the form that collapsed , chipping shall be conducted and remove loose concrete
    and laitance , conduct reinspection prior to form closure and prior to concrete pouring.
    Corrective Measures Implemented by: Michael Magat - QC Engineer

    Root Cause Analysis: Formwork is not complying with the structural calculation particularly on the spacing of Tie Rod
    that cause the collapse of the form on the North West bottom portion of the wall .
    Reinspection Required: Yes

    Reinspection Status: After the reinstallation of the forms , re inspection shall be conducted with full attention on the
    tie rod , waler and support installation.
    Preventive Action Taken: Strict compliance of the structural calculation particularly on the spacing of the tie rod which is 600 mm
    vertical and horizontal.
