1. OBJECTIVE

This procedure has been prepared to provide information and guidance for responses to emer-
gency situations. The procedures were developed to cover emergencies in general which may
occur during our day to day operations in all areas to include but not limited to: workshops of
various nature and activities, offices, construction, electrical phases of the First 250MMSCFD
Train Of KM500 Gas Treatment Plant Project. While all situations cannot be covered, the
procedures listed remain applicable and should serve as overall guidance for any given
emergency. Also keep in mind this is not an all inclusive plan. This procedure is not, nor is it
intended to be, a substitute for good common sense that must prevail in any given emergency
situation.

Each member of HAVATEK management and our sub-contractor if applicable is expected to


contribute personnel, materials and resources, as appropriate, to ensure the efficient and maximum
response to any emergency situation which may occur throughout this project site.

2. SCOPE OF APPLICATION

2.1 Scope

This procedure applies to all personnel who are actively engaged in working on any HAVATEK
operations and or site during the construction phase of the Project, up to the point when it is
handed over to the customer if applicable.

2.3 Responsibilities

Incident Manager (HAVATEK HSSE Manager in case of absence or Site Manager on the
Project).

Overall in charge of the incident and all communications with outside authority / Base Command
& Control

Subcontractor Project/ Site Manager in-charge.

To provide manpower and assets such as equipment (front-loader, trucks and forklifts), shovels,
drums etc.

Subcontractor HSE Manager/Officer (If applicable)

His responsibilities include - cordon off the areas and evacuate workers if need be, to have
accountability of those working in the affected area and secure the affected area when instructed
by Incident Manager or HAVATEK HSSE Manager.

3.1 MALICIOUS ACTS

Include occurrences such as theft and vandalism. Both HSE Manager & Country Manager will be
contacted to determine if police is needed or local tribes people.

The site will set up suitable arrangements for contact with the site medical staff, fire crew and
emergency response team: -

Mobile phone will be used (Radio at later stage maybe used). The Incident Manager will be the
HSSE Manager and he will assist by his Deputy HSSE or the Site Manager’s HSE Office will be
the command centre during emergency.

The Incident Manager will reserve the right to formally declare any emergency after assessing the
situation and as per guidance then and only then will HAVATEK external support (Civil Defence,
Police, Municipality, etc) if necessary.

5. EVACUATION ARRANGEMENTS

The Site will make the following arrangements in case of evacuation:


o The number of Muster / Assembly Points will be allocated (to be determined base on each
location site) and will be indicated on site plan.
o Simulation Exercises shall be conducted prior to activity commencing, and documented on a
annually basis or as requires.

6. EMERGENCIES

6.1 Medical

Contact the Administration In-charge, Camp Boss in-charge and an alternate (immediate
supervisor, HSE Officer) is required.

Do not approach or touch the injured person without considering the presence of toxic gases or
harmful chemical or electrical power cables nearby.

Remove the injured person from any immediate danger whilst maintaining your own safety.

Administer first aid if trained and certified to do so, otherwise wait for Emergency Action Team
members.

Make every effort to make the injured person safe and comfortable until medical help arrives at
the scene.

After assessing the situation, medical personnel will evaluate the severity and decide if the patient
requires to be evacuated to Hospital or not.

6.2 Fire

If minor fire, try to extinguish using fire extinguisher but ensure escape route away from the fire is
available at all times. If large fire or if unable to extinguish minor fire call the fire dept.

6.3 Environmental

Oil Spill or Release - ensure any Potentially Hazardous Substances (PHS) cannot enter storm
water drains, trenches, rivers or run into other sources.. Bund area off spillage as quickly as
possible using sand, oil booms or other suitable material. Use hazard-warning tape to cordon off
the area and establish as no smoking, no source of ignition zone, position foam filled fire
extinguishers close to the area if required

If a large spill or release occurs than arrange for a tanker. If it is small enough to place in drums
transfer as soon as possible. Soak up remaining PHS with absorbent material and excavate
contaminated soil if applicable. Treat used absorbent material and contaminated soil as hazardous
waste and dispose of through a licensed contractor to a licensed facility. Recovered oil should be
considered for recycling.

Chemical spills or release – If chemist is available and on site seek immediately his advice, if not
refer to Material Safety Data Sheet (MSDS) available in HSSE Office, Administration office and
Stores for information on HSE precautions and whether or not it is necessary to evacuate the area.
Wearing the correct emergency personal protective equipment and at a safe distance from the
chemical try to divert flow path away from storm water drains or collection points, and rivers.If
possible create a bund using sand, chemical booms or other suitable material to contain the
chemical spill. Use hazard warning tape to cordon off the area and have fire truck positioned close
by and if the chemical has entered storm water drain, river or sea contact the Local Emergency
Services and Environmental Protection Agency (EPA) if available. Treat and neutralize chemical
as advised by chemist or as stated on the MSDS. Arrange tanker if large spill or transfer to
suitable drums as soon as possible. Soak up remaining spillage with absorbent material and
excavate contaminated soil. Treat waste including absorbent material as hazardous dispose of
according to local municipality and excavate soil if applicable.

6.4 Bomb Threat

Terrorist threat / Bomb alert. (Try and obtain as much information as possible)
If a bomb, its exact location and time of detonation.
What demands are being made
Make a note of any background noise on the phone
Make a note of sex and appropriate age of the caller

If possible try and have someone else listen in on the call.


Once the connection has been broken do not hang up the phone.
Make a written record of the above including time of call and time of termination.
Contact the Security and Management Immediately.
Evacuate the site unless instructed otherwise by the caller or the Security or police.

6.5 Major Gas Leak

Depending upon the size, nature, location and type of gas involved, it may be necessary to
evacuate all personnel from the site as the first response to a safe distance. Muster points shall be
established for this purpose. Safe distance may change depend on the site conditions, shall be
obeyed. Should the Incident Manager take this course of action the Evacuation shall be given by
means (if depending on each site location) should be sounded and the site be evacuated, and a
head count, by the card system, and a sweep of the area.

Consideration will need to be given immediately to ensuring that personnel are moved to a
safe distance up wind of the leak or release.

6.6 Natural Disasters - Heavy Wind, Rains with Sand or Electrical Storm,

If the site is located beside the river, or Mountain side, as such windy conditions are normal but
occasionally there may be heavy wind resulting in sand or electrical storms. These winds can go
up to more than 25 knots (29 mph or 46 kph). If the wind speed exceeds more than 20 knots
(23mph or 37 kph), all lifting works by mobile crane, cherry picker works and working at heights
may be temporarily stopped at the sole discretion of the Site HSSE Manager/ Site Manager. In
extreme conditions (e.g an earthquake in the region which triggers landsides or electricl storm
forest fires alert), the site may be closed indefinitely for work activities at the sole discretion of the
Site Manager.

6.7 Confined Space

EMERGENCY 1ST AID AND EVACUATION ARRANGEMENTS

For the area(s) in questions and mentioned above will follow these established procedures

1. Watchman is to establish the nature of the emergency once this is done.


2. Then shout/whistle for any assistance he may need for help.
3. He will then call for the Ambulance which will be dispatched (If necessary)
4. Watchman if Confined Space Entry (CSE) or Designated person in Restricted Space will call
the immediately HSSE Manager or his In-charge, and then call Site or Construction Manager,
and notify him of the incident and details if possible.
5. The Dr. if required will be notified as well from Havatek.
6. Summon someone to quickly get the 1st Aid Kit if needed to administer 1st aid.
7. Next if any 1st Aid Treatment is required ONLY a Competent 1st Aid responder will treat the
injured victim and if required until Medic or Dr. arrives a person can assist the 1st. Aider.
8. Once an assessment of the condition of the injured person is determine, the means of evacuation
route are in order.

9. Then we will use a board/basket and ensure a crane also position, or the use of a tripod to raise
the injured person from the CSE or Restricted Space
10. Once loaded and raise the injured person will be transported if required by ambulance for further
assessment of offsite to a hospital.

HSE PERSON(S) IS TO STAY AT THE LOCATION OF INCIDENT AND RECORD THE


EVENT.

a) Name of victim(s) Badge numbers and position


b) What happened? (Take witness statements and ask them to provide it in their mother language
if possible)
c) When did it happen? Time and date.
d) Where did it happen? General location and exact location of incident.
e) How did it happen? Were there any special circumstances?
f) Why did it happen?
g) Take pictures of the location. More than one picture of the place of incident from different
angles.
h) Is the paperwork in order? (RTC, HRAP, TBT Gas Testing if applicable) Does the injured person
have a certificate to operate that machine or equipment (if required)? Is he qualified and fit to
do the job?

Prior to entry and/or work in the confined space:

1. The entry supervisor will ensure that an adequate number of appropriately trained persons (are
available for confined space work.

2. The entry supervisor will review all emergency procedures, including procedures relating to
emergencies outside the confined space with all entrants and other related personnel.

3. The attendant establishes communication with all workers related to emergeny situations

On entry and while working in the confined space:

1. The attendant who is stationed outside and near the entrance to the confined space remains in
constant communication with all workers inside the confined space.

2. The attendant must be notified immediately if an entrant recognizes:

- unusual action/ behaviour

- an unexpected hazard

- an unsafe act or

- detects a condition prohibited by the permit

3. Entrants must exit the confined space as quickly as possible, when:

- an order to evacuate is given by the attendant or entry supervisor

- an entrant recognizes a sign or symptom of over-exposure

- an unacceptable condition arises or

- an evacuation alarm is activated.

In the event of a confined space rescue:

1. The attendant does not enter the confined space but immediately summons a rescue response
from the rescue team.
2. HSSE manager shall evaluate the need for a rescue team depending on the Confined Space
type, nature of the work and other conditions/hazards present.

Retrieval Systems and Methods of Non-Entry Rescue Retrieval systems shall be available and
ready when an authorized person enters a permit space, unless such equipment increases the
overall risk of entry, or the equipment would not contribute to the rescue of the entrant. Retrieval
systems shall have a chest or full-body harness and a retrieval line attached at the center of the
back near shoulder level or above the head. If harnesses are not feasible, or would create a greater
hazard, wristlets may be used in lieu of the harness. The retrieval line shall be firmly fastened
outside the space so that rescue can begin as soon as anyone is aware that retrieval is necessary. A
mechanical device shall be available to retrieve personnel from vertical confined spaces more than
five (5) feet (1.50 cm) deep

6.8 Fall from Height

IMPORTANT – Rescue of an employee that has fallen must begin as quickly as possible to avoid
suspension trauma. Alternately, the onset of suspension trauma can be slowed if the employee can
stand in suspension relief straps or a loop on the end of a rope.

If an employee falls and is suspended by their fall arrest harness initiate the emergency rescue plan
by following the steps listed below:

 The site supervisor takes charge of the situation and informs Incident Manager,
 Determine the safest type of rescue given existing conditions so that the rescue team can
initiate a rescue as quickly as possible.
 The Supervisor alerts other employees of the fact that an emergency situation exists and all
work in the area should stop.
 All employees in the vicinity of the incident must stop working immediately.
 Attempt to communicate with the employee that has fallen to determine their condition and
whether they can self rescue or participate in an assisted rescue.
 Ask all non - rescue related people leave the area.
 Record the time (best estimate) when the employee fell and the time when they were
rescued. The difference is the length of time the employee was suspended

 Monitor the employee’s condition constantly. The signs and symptoms of orthostatic
intolerance that can start to be seen in 2/3 minutes include:
a) Faintness
b) Nausea
c) Breathlessness
d) Dizziness
e) Sweating
f) Paleness
g) Hot flushes
h) Skin tone may appear grey in colour
i) Loss of vision
 The Site Supervisor and Incident Manager quickly evaluate the situation to identify any
further hazards that have developed as a result of the accident.
 Identify a safe landing area either on a work platform or at ground or floor level.
 Identify any hazards in or near the landing area that must be dealt with.
 Refer to the applicable type of rescue listed below and follow the procedures for
implementing the rescue.

TYPES OF RESCUE

Self Rescue

If the employee has selected the proper fall arrest equipment, installed it properly, is using it
properly and has not been injured they should, in most cases, be able to self rescue as follows:

 Make verbal contact with the employee that has fallen to help them stay calm, establish
whether they are injured and give them guidance during self rescue.
 Employee climbs back up to the work platform from which they fell.
 Rescue team assists the employee back on to the work platform.
 Employee returns to ground or floor level. Rescue team stays with the employee in case
they are unstable.
 Have the employee checked by a qualified first aider and provide first aid if required.
 Collect all of the fall arrest equipment used by the employee and tag the equipment as DO
NOT USE. Document all of the items used as well as the employees name, the date and
time of the fall and the job being performed when the fall occurred.

Assisted Self Rescue

If self rescue is not possible an assisted self rescue will be performed by using a mechanical
assist winching system. The following procedures are to be used during this type of rescue:

 Make verbal contact with the employee to help them stay calm and determine whether they
are injured.
 Maintain verbal contact during the rescue.
 While rescue preparations are being made do the following:

o If the employees harness has suspension relief straps and they are okay tell the
employee to insert their feet in the straps in order to relieve the pressure exerted by the
harness straps on their legs.
o If the employees harness is not equipped with suspension relief straps lower a rope
with a loop at the end for the employee to use as a relief strap.
o Encourage the employee to keep moving their legs while their feet are in the
suspension strap or rope loop.

 Attach the winching equipment to a securely installed properly rated anchoring point.
 Lower the winch line to the employee.
 Have the employee grab the hook on the end of the line and securely attach it to the proper
D-ring on their harness. (If necessary provide verbal directions to assist the employee.)
 A member of the rescue team must verify that there is a positive / secure connection
between the hook and the D-ring.
 The rescue team must raise or lower the fall employee to the nearest safe work platform or
to the floor or ground level.
 Have the employee checked by a qualified first aider and provide first aid if required.
 Collect all of the employees fall arrest equipment and tag it as DO NOT USE. Document
the items used, the employee’s name, the date and time of the fall and the job being
performed when the fall occurred.
 Give the fall arrest equipment and documentation to the employee’s supervisor or manager
to have inspected for defects or damage.

Fully Assisted Rescue

These procedures are to be followed if the employee is injured and is unable to attach themselves to
the rescue system.

 Make verbal contact with the employee to help them stay calm and determine whether they
are injured.
 Maintain verbal contact during the rescue.
 Attach the winching equipment to a securely installed and properly rated anchoring point.
 Rig separate lines for rescuers to use while carrying out the rescue.
 If possible use a rescue remote connection pole to attach the winching cable or rope to the
employees harness D ring.
 If the remote connection pole cannot be used lower a rescue team member to the
suspended employee to attach the winch line to the D ring in the employee’s harness.
 Raise or lower the employee to the nearest safe work platform or lower the employee to
the ground or floor. (NOTE: Unless there is a hazard that prevents the rescue team from
doing so, any employee that is injured or has been suspended in their harness for an unsafe
period of time should be lowered to the floor or ground so that first aid and medical
attention can be accessed as quickly as possible.)
 Provide first aid and required medical attention. (Ensure that the possibility of suspension
trauma is addressed.)
 Collect all of the employees fall arrest equipment and tag it as DO NOT USE. Document
the items used, the employee’s name, the date and time of the fall and the job being
performed when the fall occurred.

 Give the fall arrest equipment and documentation to the employee’s supervisor or manager
to have inspected for defects or damage.

Ladder Rescue

If a ladder can be safely set up and the suspended person can be safely reached by a rescuer on the
ladder follow the procedures listed below:

 Set up the ladder so that the suspended employee can be reached by a rescuer on the
ladder.
 Rig separate life lines for the rescuer on the ladder.
 If the employee that has fallen is unconscious or due to injuries is unable to help during the
rescue and a second rescuer is required consideration should be given to using an
alternative rescue procedure.
 Securely attach a separate lowering / winch line to the employee.
 Rescuers on the ground, or closest safe surface, should raise the employee just enough to
allow for his / her lanyard to be disconnected and then lower the employee to the ground or
floor using the lowering line.
 Once the fallen employee has been lower to a safe surface provide any required first aid
and determine if medical attention is required to deal with possible suspension trauma.

Crane ManBasket/MEWP rescue

If Available in the incident area, Crane with with man basket or MEWP may be used to rescue the
employee after evaluation by the Incident Manager and Site supervisor.

Equipment boom height shall be considered before rescue attempt.

7. TRAINING

Each employee shall receive training regarding this plan during the new employee HSE
orientation or initial assignment.

Tool Box meetings shall be used periodically to re-emphasize the requirements set forth in this
Procedure.

Retraining will occur when the plan changes due to a change in the layout or design of the facility
and every annual as required.

All drills shall be recorded.

Emergency training for all employees should address:

Individual roles and responsibilities.

Information about threats, hazards and protective actions.

Notification, warning and communications.

Emergency response.

Evacuation.

Location and use of common emergency equipment.

This Emergency Plan and related trainings will be updated when a change in key personnel
and according to outcome of drills to ensure it is practible at all times during the project.
